A delivery truck driver was loaded with stone and pulled forward. He called his truck owner saying he was having trouble breathing. 911 was called and our employees performed CPR till life squad arrived. He was transported to Christ Hospital where he passed two days later.

Employee was using a folding knife to push the locking pin of a wiring harness. The knife slipped off of the pin resulting in a finger laceration on the left hand requiring sutures.

While operating the Hitachi EX 1100-3 shovel loading haul trucks with stripping material, the operator noticed flames on the equipment. The employee exited the cab immediately and Eagle Crushed Stone's emergency action plan was initiated. MSHA and the local fire department were notified. The Hitachi EX 1100-3 shovel sustained significant damage.

WHILE EXITING THE LOADER THE EE MISSED THE BOTTOM STEP CAUSING HIM TO FALL TO THE GROUND. THE EESTATED HE HEARD A LOUD "POP" IN HIS KNEE. THE EE DID NOT SEEK MEDICAL TREATMENT UNTIL 5-28-03 AND THE ACCIDENT WAS REPORTED 5-29-03.

EMPLOYEE FELT PAIN IN HIS LOWER BACK AFTER OPERATING A 50-TON CAT HAUL TRUCK. HE SAID HIS BACK WAS BOTHERING HIM. HE WENT TO THE DOCTOR ON 7/10/02 (LOWER BACK STRAIN.)
AFTER EE REMOVED LAST BOLT OUT OF A PAN IT SLIPPED PINCHING HIS LITTLE FINGER ON HIS RT. HAND. HIS X-RAY WAS POSITIVE FOR A SMALL HAIR LINE FRACTURE. TREATMENT CONSISTED OF TAPING ADJOINING FINGER TO INJURED DIGIT FOR SUPPORT.

EE OBSERVED A CO-WORKER HAVING TROUBLE. HE QUICKLY EXITED THE DOZER AND SLIPPED ON THE TRACK AND STRUCK HIS RIGHT KNEE.

SOMETIME DURING THE DAY EMPLOYEE GOT A PIECE OF STELL FROM GRINDING OR WELDING. HE DIDN'T NOTICE THE PROBLEM UNTIL HE WENT HOME AND SHUT HIS EYES TO SLEEP. WAS WEARING HIS SAFETY GLASSES WHEN WELDING.
