O'Connor Brothers Incorporated · Fall from machine

EMPLOYEE SLIPPED OFF EXTEC PORTABLE PLANT & FELL TO GROUND, CAUSING MULTIPLE BRUISES. A SIGNIFICANT CONTRIBUTING FACTOR WAS THE ACCUMULATION OF EXCESSIVE FROST ON THE PLANT. MULTIPLE BRUISES, EXTERNAL & INTERNAL, WERE INCURRED. THERE WERE NO BROKEN BONES OR HEAD INJURIES.
