EE reports was assisting install a tail (bin) pulley out of a manlift along with a second employee. The second employee was using a come-along to lift the pulley up so they could get it in position when the EE decided to assist by pushing the pulley. It weighs approx. 100-150 lbs. As the EE began to push, EE felt pain in lower back. EE started PT on 11/14/2024.

EE was replacing a top roller on feed belt when EE encountered resistance because of bent ear on the roller carriage. EE used a pry bar to help push the roller into place. The pry bar slipped out and jerked EE's shoulder. The pry bar did not strike employee. Contacted WorkCare. EE was determined to need surgery-9-2-22.

Employee initially injured L shoulder when shoulder struck a beam when pry bar slipped. Checked out and released without restrictions. Early September reported continued discomfort. Medical evaluations reveal injury to rotator cuff. Specialist visit on 9/17 suggests surgery on the shoulder on 10/2/2020.

The injured party had completed fueling the haul truck at the end of shift, stepping off curb at fuel point, employee twisted/sprained knee. Employee received diagnosis in September after the knee got sore. Employee continued to perform regular job up until employee had arthroscopic surgery on January 22, 2019.

The lid on a grease drum had a metal tab sticking out on one side. Employee was changing out grease barrel when EE's left elbow contacted the metal tab. The result was a laceration requiring six stitches to close. No lost time and no restricted duty.

Released to regular duty that day but was prescribed physical therapy; thus this report
