The employee was mounting a D11 Dozer, when EE lost EE's balance reaching for the grab rail, falling off the track to the ground.

The Operator was notified on 11/25/2025 that the employee has been diagnosed with Occupational Pneumoconiosis.

EMPLOYEE WAS CLIMBING OFF A D10R DOZER AND MISSED THE BOTTOM STEP ON THE PUSH ARM TWISTING RIGHT KNEE

The employee reported chest pains to EE's co-worker and subsequentially lost consciousness. EMTs treated the employee who regained consciousness. The employee was transported to the hospital where EE was diagnosed with arrhythmia and anxiety. EE was treated and released and returned to work on EE's next regularly scheduled shift. This was a medical event, not work related.

The injured employee was in a CAT785 Rock Truck that was being loaded by a CAT993K Loader. The loader ran over a rock causing the loader bucket strike the truck bed. This caused the injured employee to be rocked back and forth, causing neck pain.

Employee was walking up the stairs on EE's truck when EE felt a pop in EE's right foot. EE was diagnosed with a non displaced fracture of the 5th metatarsal bone.

Employee fell while walking around hill to cut a tree, landing on the chainsaw they were using, resulting in a laceration to the left arm. Employee received eight stitches and returned to work the next day.

Employee was descending the Pax haul road at the #7 marker, EE stated EE lost EE's brakes. EE then guided the truck which was loaded into a sump causing the truck to stop. EE struck face, head on the steering wheel causing a broken nose and right eye orbital fracture. EE was wearing seatbelt and road conditions did not play a part in the accident.

A hose ruptured, spraying flammable liquid on the turbo charger causing an equipment fire. The operator exited the loader. There were no injuries.

EE was shutting the door of a dozer that the EE was operating. The EE pulled the door closed, smashing left middle finger. EE received (4) sutures, and returned to work the following day.

Employer was notified on 3-27-23 the employee was diagnosed with occupational pneumoconiosis.

Employee was entering cab of Highwall Miner, the wind caught the door and slammed the door shut. The employee's hand was caught between the door and door frame.

employee was arc welding on the bucket of a large excavator. It appears that a stray spark melted a hose resulting in fire. MSHA was notified of a fire, an inspector responded. The sole purpose of this report is due to the fire. No one was hurt.

Employee was operating MT5811 rock truck when EE experienced pain in EE's lower back.

Tension spring came off of brake pedal. While tramming to park equipment, pedal fell to floor causing brakes to fully engage. Employee's upper body thrown forward while seat belt kept lower body stationary creating whiplash effect.

Putting plate against roller frame then placing hydraulic ram under plate. Leaned over and arm contacted control button for ram causing it to extend, catching right thumb.

Installing fan blade on D10T tractor cut left thumb joint with fan blade

PM Tech was servicing D10R dozer when the Tech lost balance. Grabbed ahold of dozer with right arm to regain balance. This resulted in twisting of left knee and feeling a pop in right shoulder causing pain to both.

Employee standing beside 988F loader holding grease hose on fitting, co-worker moved bucket causing right forearm to be caught in pinch point.

Employee accidentally ingested diesel fuel. The employee was diagnosed with toxic effect of chlorofluorocarbons, accidental (unintentional). This is a degree 7, classification 24 occupational illness.

While pulling down a ratchet tightening 993k front wheel unit mounting bolts at Simmons Fork straining lower back. Employee did not seek medical attention until 10/24/17 and didn't send in paperwork until 11/1/17.

Employee was driving in a pin on a dozer push arm when a piece of the pin chipped off and struck the employee's eye.

Employee was dismounting man-trip. The employee missed the last step resulting in the twisting of the ankle. The employee went to a medical facility, x-rays were negative. The employee returned to work on next shift. On Nov 20 a fracture was noted. The employee was already scheduled off for Thanksgiving Holiday so first shift missed was on 11/28/2016.

Tech was kneeling down welding and a piece of slag came over safety glasses and into eye.

Employee was operating a dozer when rocks fell striking and jarring the operator.

Employee was breaking down track on a dozer. While knocking pin out, employee was holding onto the upper track. As the pin released, the track shifted pinching the outside of the left hand. The employee had normal days off on June 11 and June 12. Employee has missed no days to date.

Two employees were loading a D9 valve chest into a service truck. As the chest was repositioned on the truck, one of the employee's had a finger pinched between the valve chest and the service truck bed. The employee suffered a laceration requiring 3 sutures.

A 993 K model loader caught fire while operating. The fire may have begun in an engine compartment behind the operator. After seeing fire the operator lowered the bucket, shut the engine down and exhausted the fire extinguisher. He then exited the equipment. A brush fire began from the equipment fire. No one was hurt during the event. The fire department extinguished the fire.

Employee presented with difficulty breathing and appeared to be in and out of consciousness. On site EMTs provided care by placing employee on oxygen and transporting. ER staff determined the employee had a seizure. We feel the case should be degree 10 as he had a seizure and are reporting based on employee being described as in and out of consciousness.

On Nov 21 we were notified that the employee was diagnosed with occupational pneumoconiosis.

EE on the ground changing batteries. Co-worker started the 785C Service Truck and got out of the cab to tell him to turn off the welder. The 785 rolled back and trapped his right leg and foot between the bumpers of the trucks. Co-worker got back into the cab and put the 785 in reverse to get the bumper off of his right leg and foot. He took injured EE to the hospital immediately.

Left foot rolled. Fell on Rt. Knee. Twisted up & under.

Employee was striking the lipsaver of the PC3000 Bucket with a sledge hammer during maintenance procedures and a piece of metal sheared off the lipsaver and penetrated an area of his lower left leg.

Rock truck was being loaded by a PC 3000. Material was in bed of truck next pass a rock rolled from the bucket striking left front corner bed jarring operator. Mine operator has conducted a diligent investigation this time are reporting as a lost workdays (NFDL) accident, pending the review of additional med info. We reserve our right to amend, if necessary.

The EE began U.G. mining in 1975 and first filed for Coal Workers Pneumoconiosis in 1987 and was awarded 5%. The EE switched to surface mining in 1990. He was awarded a total of 20% and we were notified of that fact on 8/23/12. The EE has had no lost time or restricted duty.

Cut thumb on jagged sheet metal while installing engine enclosure on D11R. Required 8 stitches to close laceration.

EE & one person loaded a jack pad from a surface drill onto the walkway of the drill as the pad had came loose from the jack. The drill operator then moved the drill to wait on a mechanic. Then he pushed the pad from the walkway so it would fall to the ground. As the front edge of the pad fell, the rear raised & pinched the thumb between the pad & a hand rail. 5 sutures needed.

Right ring finger caught between 992G center drive shaft and steering jack boss on frame. Fractured right ring finger.

EE had been sick all morning and was vomiting, he had decided to go home if he vomited one more time. He pulled to the side of the road to vomit and when he leaned over the handrail, he apparently blacked out and fell 11 feet to the ground.

He was working on a dust collector. When he stepped onto the deck, his foot slipped, and knee made a popping sound. It was raining at the time, so deck was wet.

Fell backwards out of back of service truck while stepping to bumper.

Employee was stooping over D11R roller frame, cutting pins out of major boggle. He stood up and felt severe pain in lower back. Employee began missing work on 10/26/2009.

Employee was climbing off of powder truck when he stepped on frozen uneven ground causing him to fall on left ankle.

lifting an emulsion pump hurt back.

Tightening roller bolts on a D11R with a ratchet and prybar, felt a pop in right forearm.

Unloading oil filters into customers oil refuge bin, when one of the filters came loose smashing right pinky finger into the metal bin causing fracture.

Technician was replacing bucket teeth on a 992G. Technician was using a sledge hammer on the pin driver when a small piece broke off the end of the pin driver and lodged in his right hand between the knuckles of his first and middle finger.

While dismounting an endloader, Employee alleges that he slipped & fell- thus injuring his back. Management questions this accident. This mine was idled the week of Thanksgiving.

EE WAS LOADING A LARGE ROCK WHEN THE ROCK FELL BACK INTO THE BUCKET, JARRING HIM. EMPLOYEE DID NOT MISS WORK UNTIL 8/8/05, WHEN HE SOUGHT MEDICAL ATTENTION.

Employee was climbing down off of a Caterpillar 992G and as he was climbing down he grabbed hold of a hose as a hand hold instead of the ladder. In doing so, he tore, possibly fractured, left shoulder.
