38920ACCIDENT OCCURRED AT AN UNDERGROUND TUNNELING OPERATION AT A SURFACE LIMESTONE QUARRY. A SHOT WAS BLASTED INSIDE THE TUNNEL AT 12:04PM, THEN VENTILATED. Employee was performing a team lift with another EE to set a 100lb. LP bottle from the back of a service. Due to a miscommunication between employees, injured employee had EE's fingers under the bottle when it was set down on the bumper.-smashed the tips of the middle and ring finger of left hand - resulted in restricted duty
To maintain safe access, employee was cleaning undercarriage on excavator with a short-handled scraper. As employee was cleaning, they struck steel and jarred their shoulder. Employee was seen at a clinic, returned to work full duty, after a period of improvement, on 2/2, clinic referred employee for physical therapy. continues to work full duty, but PT makes it an MTI

Employee was cleaning a drain trough. when they went to replace the grate (80#) over the trough, they felt a strain in left shoulder. Reported to supv. Contacted clinic, who advised RICE and OTC ibuprofen. No restrictions at that time. At follow appt. 5/28, placed employee on weight restriction R arm- first day restricted 5/28/25

Employee sustained pin-hole type lacerations on EE's abdomen caused by ruptured ANFO loading hose.
Employee's finger was pinched between hoist chain and cute while maneuvering a 16"x48" piece of steel into chute. The contact resulted in the employee receiving a fracture and laceration with 12 stitches to the right index finger.

Lifting a screen panel with right arm outstretched below waist. As EE was lifting the panel, EE heard a pop in the right arm below the bicep and above the elbow. Employee used first aid processes until the arm started bruising. On 10/9/2023 EE was scheduled for an MRI. The MRI showed a torn Bicep on the right arm. Employee was then scheduled for surgery on 10/13/2023.

A miner suffered an irritation to right eye while using an oxy/fuel gas torch. The injured miner was working overhead cutting steel and some debris dropped behind the miner's safety glasses. The miner was provided RX ophthalmic ointment to treat the irritation.
A mechanic suffered a unilateral inguinal hernia to EE's right lower abdominal area while installing a park brake assembly on the mechanic's truck. The injured was in a squatted position and was using a ratchet wrench to tighten the bolts on the brake assembly. While tightening the bolts EE felt a pull in EE's right side.

A millwright suffered a laceration to EE's left lower leg (shin) requiring sutures when EE struck it against a conveyor frame. The injured was walking on the conveyor walkway and stumbled causing EE's left shin to contact an angle on the frame as EE caught self. The injured states EE thinks the tread on EE's boot caught on the grating causing EE to stumble.
A Mine Engineer sustained a fracture to right lower leg while operating the JLG Manlift. While tramming the manlift drove over a rock which caused them to unexpectedly get jostled in the basket, causing them to come down on right foot in an awkward manner resulting in the injury.
A mechanic suffered a strain to right shoulder while working on the Scaler. The injured miner was attempting to tighten a fitting on a 2" hydraulic hose when the tool EE was using slipped and EE felt a strain in the shoulder.

A Miner suffered a strain to EE's right knee while traveling by foot. The miner states EE slipped but did not fall while carrying a 5 gallon pale of oil to the E5 gearbox. EE has been placed on work restricts after a follow up visit to the clinic.

A mechanic sustained an injury to the tip of ee middle finger on ee right hand while replacing the hammer on the rock breaker. The finger was injured when it was caught between the hammer and pin as ee was attempting to line up the pin.

Employee contacted supervisor and stated that employee had gotten something in right eye the shift prior. Employee stated was unaware of when or where it had happened. Employee stated employee wiped eye and felt fine and completed shift. That night when employee removed contacts felt irritation in right eye. Incident was not reported on that date it happened
Employee stated that EE had taken the Surf Yard truck to retrieve EE's harness. EE stated EE stopped truck outside portal & got out to pick up EE's harness which EE stated was inside portal. Employee stated EE walked behind truck to retrieve harness & noticed truck beginning to move forward. Employee stated EE received injuries while attempting to stop truck
