Employee was loosing a bolt on a hammer attachment mounted to an excavator. Lost EE's balance and fell, fracturing EE's left wrist.

EE was using a cordless drill and a 3/8 drill bit. EE was drilling a hole in a 1/8 inch thick aluminum mounting plate when the drill bit caught causing the drill to grab and twist in EE's hand.

EE stepped on a board and twisted EE's ankle. After being seen at the doctor's office it was determined that a bone in EE's ankle was broken resulting in restricted duty.

EE claims that the brakes failed at the top of the slope leading into the pit. EE was on his way to get his 9th load of the night. EE had conducted pre-shift on the haul unit. The vehicle str4uck the highwall as he attempted to negotiate a curve and ramp leading out of the pit.EE received a laceration to left forehead area and was hospitalized.

HE WAS SEEN BY CO-WORKERS AND THE BELT WAS STOPPED.EE SUFFERED BRUISES AND A BROKEN LEFT ARM. EE CLAIMED IT WOULD TAKE TOO MUCH TIME TO LOTO TO

EMPLOYEE WAS EXITING HIS MACK TRUCK WHEN HIS RIGHT FOOT SLIPPED OFF THE TRUCK STEP. THE EE FELL TO THE GROUND, TWISTING HIS RIGHT ANKLE IN THE PROCESS. EE WALKED TO A NEARBY CONTROL HOUSE BEFORE HE NOTICED SWELLING IN HIS RIGHT ANKLE. X RAY EXAMINATION REVEALED THE ANKLE WAS BROKEN.

EE WAS STANDING AT EMPLOYEE BUILDING DOORWAY WITH HIS LEFT HAND ON THE DOOR JAM. A CO-WORKER ENTERED THE BUILDING THROUGH THE DOOR AND THE EE'S LEFT LITTLE FINGER SLIPPED INTO THE HINGE AND WA S PINCHED OFF AS THE DOOR SLAMMED SHUT. LEFT LITTLE FINGER TIP WAS CRUSHED OFF TO THE FIRST KNUCKLE.

INITIAL CHIROPRACTIC DIAGNOSIS FROM EE'S PRIVATED DR. INDICATED COMPRESSED DISKS. FURTHER MEDICAL EXAMINATION DISPORVES THIS INITIAL DIAGNOSIS.

DURING THE MORNING WORKPLACE INSPECTION A GROUND FAULT WAS DETECTED IN THE PRIMARY CRUSHER TOWER SYSTEM. IN THE TROUBLE SHOOTING THE PROBLEM THE EMPLOYEE NEGLECTED TO FULLY LOCK OUT ALL POWER SOURCES AND RECEIVED AN ELECTRICAL SHOCK AND BURN TO HIS RIGHT MIDDLE FINGER. HE WAS PULLED FREE OF THE POWER SOURCE AND TAKEN DIRECTLY TO A HOSPITAL.

THE UNIT PROCEEDED TO TIP STRAIGHT UP AND LANDED O N IT'S TAIL GATE. BY RAISING THE BOX THE EMPLOYEE RIGHTED THE UNIT. HE SUFFERED A SLIGHT CONCUSSION FROM HITTING HIS HEAD ON AN UNKNOWN OBJECT.

EMPLOYEE WAS ATTEMPTING TO OPEN VENT WINDOW ON A STOCK PILE TRUCK. THE LOCKING DEVICE ON THE WINDOW WAS RUSTED AND UPON TRYING TO LOOSEN AND MOVE IT, THE LOCK BROKE OFF LEAVING A SHARP POINT. THE EMPLOYEE'S ARM CONTINUED THROUGH THE WINDOW ACROSS THE BROKEN REMAINS. THE RESULTING ARM LACERATION REQUIRED 13 STITCHES.
