Flint Ridge Mine: 645 MSHA citations Coal

The above mentioned mine has experienced a non-injury, unintentional roof fall outby the active workings of the mine. The fall occurred in #5 entry off 1st right mains, old works. Fall was 30 feet long, 20 feet wide and app. 6 feet thick, shale. 48 Inch fully grouted rods were initially used. area dangered off and cribs installed.

The above mentioned mine has experienced a non-injury, unintentional roof fall on the #5 conveyor belt at x-cut #10. The fall strata was shale, app. 30 feet in width, app. 23 feet in width and 4 feet thick. 48 Inch fully grouted rods and 10 foot cable bolts was the initial support.

The above mine has experienced a non-injury, unintentional roof fall at cross-cut no. 36 thru a cross-cut between no. 3 and 4 entries along no. 3 belt mains. The fall was discovered at app. 4:30 am during a pre-shift examination. The fall is app. 5 feet thick, 20 feeet wide and 45 feet long.

A ROOF FALL OCCURRED 1 BREAK INBY SPAD #257 IN THE NUMBER 2 ENTRY ADJACENT TO THE NUMBER 2 BELT APPROXIMATELY 20' WIDE X 40' LONG X 10' HIGH

FALL OCCURRED 5 BREAKS INBY THE #2 HEAD DRIVE IN THE #6 & 7 ENTRIES IN THE INTAKE ENTRIES. THE FALL WAS 10' THICK, 80' LONG, AND 20' WIDE. THIS AREA HAD BEEN SUPPORTED WITH 4' FULLY GROUTED ROOF BOLTS.

A ROCK FALL WAS DISCOVERED ON 9-03-08 ADJACENT TO THE #2 BELT AT THE 8TH XCUT BETWEEN THE NUMBER 2 AND 3 ENTRIES APPROXIMATELY 8' THICK 20' WIDE AND 60' LONG.

EE WAS CHANGING BITS IN THE HEAD OF THE CONTINUOUS MINER, WHEN HE STRUCK HIS THUMB ON HIS LEFT HAND WITH A HAMMER, RESULTING IN A LACERATION THAT REQUIRED 3 SUTURES.

A ROOF FALL OCCURED AT THE 9TH XCUT ON THE #3 BELT IN THE NUMBER 5,6 AND 7 ENTRIES. THIS FALL OCCURRED SOME TIME ON THE 3RD SHIFT AFTER COAL PRODUCTION CEASED ON THE 2ND SHIFT. THE FALL MEASURED 20' LONG, 68' WIDE AND 5' TO 8' THICK.

A FALL WAS DISCOVERD ON 8-4-2008 IN THE #6 ENTRY ADJACENT TO THE #2 BELT AT THE #13 CROSS CUT, THE FALL WAS 20' WIDE 30' LONG APPROXIMAATELY 6' THICK.

A ROOF FALL WAS FOUND IN THE RIGHT RETURN (#8 ENTRY) AT 8:55 AM THIS WAS OFF THE THE #2 BELT. THE FALL WAS APPROXIMATELY 10 FEET WIDE, 10 FEET LONG, AND 4 FEET THICK.

EE was in the process of hanging a block curtain across the belt out by the feeder when a belt splice caught the curtain and jerked him down resulting in a fracture to his left ankle.

Employee was cutting a piece of ventilation curtain with his knife and accidently cut his forefinger.

Employee was in the #4 entry helping the miner operator move the continuous miner when he stepped into a hole and turned his left ankle. This section had moved to a new mining area and was experiencing bad bottom conditions.

A rock fall occurred in the number five (5) entry two (2) breaks outby the number one (1) tailpiece at 2:00 PM on 9-19-2007. The top in this area was shale and had been supported with four(4)foot grouted rods.

EE was on the section mantrip in the #4 entry adjacent to the #3 belt traveling toward the section, when the rubber tire mantrip hit a hole and caused him to absorb the shock, resulting in an injury to his lower back and right knee. EE had an injury on his right knee on 7/28/07 and an injury to his lower back on 4/3/07.

EE was in the process of hanging the continuous miner cable in the number five (5) entry when he felt pain in in his lower back. He has been having back problems prior to this incident.

The roof bolter operator was preparing to drill a hole to install a roof bolt. As he started to rotate his drill steel his glove got caught on the drill steel causing his left hand to be wrapped around the steel resulting in a laceration to the little finger and ring finger.

The miner operator was cutting coal in the #5 entry and was getting ready to start his clean-up run when a piece of draw rock fell from between the roof bolts hitting him on the head and left shoulder. He was standing three rows of bolts from the unsupported top. Employee did not start missing work until 3/27/07.

Employee (roof bolter operator) was preparing to drill a hole in the roof for a roof bolt when his foot slipped under the drill boom while he was lowering the boom, catching his foot. The area was dry with a small amount of loose coal.

Employee was removing the lid off of a shuttle car electrical panel and he dropped it on his foot.

Ee stated that he was pulling jumper cables at the 5-A station for his ram car when he felt pain in his lower back. (This accident was not reported until 9/5/06).

Employee was repairing a hole in an overcast when a piece of metal fell and he tried to catch it and cut his middle finger on his right hand.

Injured was struck on the head by an empty glue box when employee threw it across the roof bolt machine toward the scoop bucket. Injured complained of being dizzy and sick to his stomach.

EE was pulling jumper cables at the ram car charging station when he felt pain in his right arm, above the elbow.

EE WAS USING A HAMMER TO BUST A PIECE OF ROCK ON TOP OF THE CONTINUOUS MINER. A PIECE OF ROCK FLEW OFF STRIKING EE IN THE GROIN AREA.

Employee stated that he bumped his left leg below the knee on 3/27/06 on a piece of belt structure and it made a blue place. He said that he did not think it was anything but it got worse over time. When he came outside after working the third shift on 4/10/06 he showed his leg to the mine superintendent and told him what had happened. He told him to go to the doctor.

EE stated that he started to turn around, and he felt pain in his left knee. This ee stated that he has had cartilage removed from both knees when he was in grade school. He is schedule to have an mri on 12-16-05.
