EE was on a grader doing a survey of the unit hours. He missed the handrail and fell to the ground. He suffered a bruise and swelling of the right shoulder, hip and knee.
While using a pry bar to place a belly pan back on D-11 dozer the bar slipped striking ee on left ear.

EE WAS DISMOUNTING HIS DOZER. HIS FOOT SLIPPED OFF THE STEP, CAUSING EE TO STRIKE HIS LEFT ARM ON PUSH ARM.

EE WAS USING A HAMMER TO DRIVE A BEARING RACE FROM SHAFT. WHILE DRIVING RACE A PIECE OF METAL STUCK HIM IN LEG ABOVE RIGHT KNEE. HE RECEIVED STITCHES.
OPERATING A DOZER OPERATOR RECEIVED A SUDDEN JAR WHICH RESULTED IN PAIN IN LOWER LEFT BACK. (THIS ACCIDENT IS STILL UNDER INVESTIGATION.)
OPERATOR WAS STANDING ON DRILL DECK CLEANING WINDOWS ON DM-45 INGERSOL HIGHWALL DRILL WHEN PULL UP CHAIN BROKE STRIKING HIM. HE SUSTAINED CUTS TO FACE AND A BROKEN LEFT ARM. HE WAS ADMITTED TO A.R.H. HOSPITAL FOR OBSERVATION.

EE WAS ATTEMPTING TO MOUNT HIS DOZER WHEN HE DEVELOPED A PAIN IN HIS RIGHT SIDE OF HIS RIB CAGE. HE CONTINUED TO WORK UNTIL 05-06-02. HE WAS OFF WORK FOR 2 DAYS.

EMPLOYEE STATED THAT AS HE WAS LOADING A BICKET OF ROCK, THE BUCKET HIT A ROCK, CAUSING HIM TO BE THROWN AGAINST THE SEAT BELT, JERKING HIS LOWER BACK. HE WENT TO THE DOCTOR ON 2-26-01 AND IS OFF WORK UNTIL FURTHER NOTICE PENDING MRI.
