Employee was assisting putting a cutting edge on a loader bucket. While raising the edge on a floor jack the cutting edge became unbalanced falling off the jack, employee's finger was under the edge when it fell. Employee required 4 stitches on right hand middle finger. Also the tip of the finger was fractured.

After performing maintenance on a CAT 775D haul the employee was cleaning EE's tools when EE noticed that something was in EE's eye. After more than one attempt at flushing the eye the employee was seen by a physician who then prescribed antibiotic eye drops as a scratch on the cornea of the eye was found.

Employee got out of haul truck on the drum ledge in the pit to move a basketball sized rock out of the way as it was in the way of the haul trucks. As employee rolled the rock out of the way, felt a pop in left arm. Reported that they could not move left arm without pain. MRI shows partial tear of bicep tendon, no surgery or rehab required.

On July 15, 2016 employee first reported that they have carpal tunnel in wrists from driving a haul truck and it progressed since November. The injury was evaluated and is now considered workers comp. Employee's first day out for surgery is 10/10/2016.

EE was contacted via radio by the leadman that the 834B had broken the drive shaft. The EE returned to the machine and placed a bar on the steering cylinders and ratchet strap around the bar and the drive shaft. While attempting to position the drive shaft for re-assembly the strap and bar fell causing the drive shaft to fall onto the right ring finger severing the end.
EE was traveling in a Cat 990 loader he stopped machine & contacted manager indicating that was injured. Manager found EE standing on loader outside cab & complaining he was in significant back pain & needed medial attention. EE was taken to hospital and referred to Occ Med facility. X-rays indicated no injury EE is seeing person physician while remains off work.

Operator heard a loud pop. When he got out to investigate he found the drill had caught fire. The on-board exting. failed to discharge and a nearby haul unit stopped to assist. The fire was too large and the EE's backed away and called the local fire dept. The fire which lasted slightly less than 30 minutes. A blown hydraulic line was likely the source of the fire.
EE was changing screens in a 6' X 16' screen deck. He attempted to move the top screen using a bar inserted down through screen. When he pulled on the bar he felt a sharp a pain in his low back. he was diagnosed with a muscle strain.
EE was poked in finger by small wire strand when she was checking the engine oil as part of the pre-operation inspection. The wire was a frayed strand of the cable type dipstick. EE was seen by a doctor and prescribed an oral antibiotic medication.

Employee climbed down a ladder at the main plant. When he got to the bottom a rock from above fell approximately 5' and struck him on the hard hat. Employee complains of neck pain and stiffness. The rock was approx 5 inches.
EE had a 25 dB or greater shift in his hearing in both ears.

Employee was climbing into the cab of a piece of equipment when he struck his head on the frame of the cab door. He worked the rest of the shift without telling anyone until the end of his shift. He said he had some slight neck pain but did not require medical attention.

EE was getting back in the truck. While climbing the access ladder he lost his grip at the third step (approx. 3') falling backwards to the ground and landing on his back, striking his right elbow on the ground causing it to swell instantly. EE was taken to Immanuel hospital in Omaha where x-rays indicated a broken elbow requiring surgery.
EMPLOYEE WENT AROUND A CURVE & HIT A BUMP IN THE ROAD & JAMMED HIS BACK. EMPLOYEE STATED THAT THE SEAT ON THE LOADER HIT HARD. IT WAS DETERMINED ON 04/19/04 THAT THIS MAY BE A COMP. INJURY SO I AM REPORTING IT TODAY.
