THE EMPLOYEE ENTERED A LOG WASHER AND DID NOT FOLLOW SAFETY PROCEDURES BY USING A LOCK OUT TAG OUT PROCEDURE. THE MACHINE WAS INADVERTENTLY ENERGIZED WHICH IN TURN RESULTED IN HIS DEATH.
