ALLEGEDLY DROVE TRACTOR ACROSS UNEVEN GROUND CAUSING PAIN TO TAILBONE. TWO DAYS LATER HE ALLEGEDLY WENT TO DR AND RECEIVED PRESCRIPTION MEDICINE. ON 8-30 WE WERE MAD AWARE ABOUT EE RECEIVING P RESCRIPTION MEDICATION ON 2-23. ALLEGEDLY ON 5-30 EE RECEIVED MEDICAL TREATMENT.

HE WAS DRIVING A DUMP TRUCK HAULING CLAY FROM THE DENTON MINE TO THE GLEASON PLANT. HE WAS CLIMBING BACK INTO THE TRUCK. WHILE CLIMBING, ONE OF HIS FEET SLIPPED OFF OF TOP STEP AND CAUSED HIM TO FALL BACK ONTO THE GROUND BELOW(APPROX 24'-30"). INJURED BACK BUT, PAIN IN HIPS, MIDDLE OF UPPER AND LOWER BACK AND SHOULDERS. NO WITNESS TO ACTUAL ACT, BUT EMPLOYEES WERE PRESENT TO ASSIST

HE WAS DRIVING AN EMPTY DUMP TRUCK, GOING TO THE DENTON MINE TO GET A LOAD OF CLAY. THE ROAD WAS A LITTLE ROUGH COMING OFF THE HIGHWAY INTO THE MINE AND IT JARRED HIS BACK. BACK STARTED HURTIN G BUT HE FINISHED THE DAY.
