Edwight Processing, LLC · Contact with electrical current

SUBMITTED UNDER PROTEST. EVENT WAS NOT ON AN MSHA ID. On July 23, 2023, other entities found the body of a trespasser at a substation not on mine property and contacted regulatory agencies. When Sundial personnel arrived onsite, police, fire, EMS, MSHA, OMHST, and the coroner were already present. Sundial cooperated with the investigations though the site was not on mine property.

EE was working at the Goals mine location cutting brush when a stick struck EE in the eye. EE claims EE was wearing a hard hat with face shield. EE's supervisor transported EE to Beckley General hospital in company vehicle. Beckley General transported EE to Ruby Memorial hospital via ambulance.

EE went up a 12 foot fiberglass ladder to perform work on ceiling area. After assesing how to tie off EE started back down the ladder, he lost his balance and fell about 8 feet landing on his back. He was transported to the hospital via ambulance where it was found that he had two cracked vertebrae and was suffering from a concusion.

EE WAS HELPING REMOVE A 25 HP 284T, 150 LB MOTOR FROM THE REFUSE SCREEN. HE WAS STANDING NEAR THE BASE OF THE SCREEN TO REPOSITION THE BOTTOM AS OTHER USED COME-ALONGS TO LIFT IT UP AND AWAY. AS THE MOTOR MOVED THE HOOK PULLED THROUGH THE EYE BOLT IN THE MOTOR, ALLOWING IT TO DROP TO THE SCREEN BASE. EE'S HAND WAS CAUGHT BETWEEN THE MOTOR AND THE FRAME.
