Employee was helping with a repair of a broken gearbox. A sledgehammer was brought up to strike the gearbox to facilitate its removal. During this process the hammer head stuck a cap extension on the gearbox. A piece of metal was ejected and struck the employee in the mouth cutting their upper lip and breaking/chipping 2 of their front teeth.
