THIS EE WAS LYING AN A CONVEYOR BELT/BIN, WATCHING EE CLEAN OUT ANOTHER BIN, THE EES POWERED UP THE BIN THEY HAD BE CLEANING WHEN THE CRUSHER OPERATOR HIT THE WRONG BUTTON CAUSING EE TO FALL O FF OF THE BELT TO THE GROUND. EE SUFFERED A COMPRESSED VERTEBRA IN THE MIDDLE BACK.

EMP WAS USING A METAL BAR TO LIFT UP A PIECE OF METAL AROUND THE HAMMER OF A CRUSHER WHEN THE BAR SLIPPED CAUSING THE WEIGHT OF THE OBJECT BEINGLIFTED TO COME DOWN ON EMP'S THUMB.
EE WAS LOWERING THE LIGHT TOWER ON THE LIGHT GENERATING UNTIL AND DID NOT USE THE SAFETY LOCK. THIS IS A NEW PIECE OF EQUIPMENT AND THE EE WAS NOT FAMILIAR WITH THE SAFETY LOCK.
EE WAS GETTING OFF OF A HAUL TRUCK BY USING THE LADDER WHICH WAS PROPERLY SECURED. WHEN HE WENT TO PUT HIS LEFT FOOT ON THE GROUND HE TWISTED IT.
