EMPLOYEE WAS IN HIS PICKUP WHEN TRUCK ARRIVED TO BE LOADED WITH LIME. EMPLOYEE DROVE PICKUP AT HIGH RATE OF SPEED TOWARDS THE AREA OF THE LIME PILE, STRUCK TWO STOCKPILES OF FROZEN MATERIAL AN D ROLLED PICKUP TWICE. IT HAS BEEN DETERMINED EMPLOYEE SUFFERED A STROKE PRIOR TO ACCIDENT, CAUSING ACCIDENT.

EE was breaking limestone boulders with 231 Excavator & hammer attachment & had the upper portion of the windshield open. As he was hammering on a rock, a 3"x1" piece flew at him & hit him in the mouth. It cut his lip open & he had a sore mouth from the impact. He drove to doctor to have it checked out. Dr said teeth ok but put 2 stitches in lip, which made this reportable.

EE WAS TOWING IMPACT CRUSHER DOWN RAMP WHEN TRACKS BEGAN TO LOOSE GRIP ON FROZEN GRADE. DOZER SLID TO SIDE AND JACK-KNIFED THE DOLLEY BECAUSE OF MOMENTUM OF CRUSHER. AFTER GETTING DOZER STOP PED OPERATOR GOT NERVOUS & BAILED OFF MACHINE INA HURRY AND LOST HIS BALANCE, FALLING TO THE GROUND, LANDING ON BOTH HANDS TO CATCH HIS FALL. LT. WRIST BROKEN, RIGHT WRIST SPRAINED.
