EE WAS HOLDING A PIECE OF CONVEYOR BELT ATTEMPTING TO CUT IT WITH UTILITY KNIFE. AS HE PUT DOWNWARD PRESSURE ON THE KNIFE TO CUT, HE LOST HIS FOOTING IN THE GRAVEL HE WAS STANDING IN CAUSING THE KNIFE TO CUT HIS RIGHT THUMB & INDEX FINGER.
EMPLOYEE WAS BINDING DOWN A PIECE OF EQUIPMENT ON THE LOW BOY USING RATCHET TYPE BINDERS. ONE OF THE BINDERS SLIPPED AND HIT EMPLOYEE UNDER THE CHIN CAUSING A LACERATION THAT REQUIRED 3 STITCHES.

EE'S LEFT LEG WENT THROUGH THE SPACE BETWEEN TWO PIECES OF THE WALKING SURFACE. HIS LEFT LEG RECEIVED ABRASIONS AND A LACERATION. DUE TO THE DEPTH OF THE LACERATION, THE DR. RECOMMENDED HE TAKE OFF THRU 1/10/03.

EE REMOVED THE GUARD FROM THE TAIL PULLEY AND ATTEMPTED TO CLEAN MATERIAL FROM THE TAIL PULLEY WHILE THE CONVEYER WAS STILL RUNNING. THE SHOVELHE WAS USING WAS PULLED FROM HIS HANDS AND HIS R IGHT ARM HIT THE CONVEYER FRAME CAUSING ABRASIONS AND LIGAMENT DAMAGE.

EMPLOYEE DELIVERED A DOZER TO PLANT SITE. TRUCK GO STUCK IN THE MUD. DOZER WAS USED TO PUSH THE TRUCK AND TRIALE OUT OF MUD. AT FIRST CONTACT INJURED EMPLOYEE WHO WAS DRIVING THE TRUCK WAS NOT PREPARED FOR DOZER TO PUSH HIM OUT. HIS NECK BEGAN TO BOTHER HIM A COUPLE OF DAYS LATER.

54736YEE WAS OPERATING A LOADER R-22 ON THE PIT ROAD.HE WAS MEETING ANOTHER R22 WHICH WAS EMPTY.EMPLOYEE MOVED OVER TO FAR TO THE RIGHT AND THE WHEELS RAN UP A SLOPE WHICH CUASED THE TRACK TO 25364E UNBALANCED.TRUCK FLIPPED OVER.EMPLOYEE SUSTAINED A BRUISED ARM A BRUISED RIB AND A LOWER BACK INJURY.THE TRUCK WAS NOT DAMAGED.
EMPLOYEE WAS ASSISTING IN LOADING OF A 30 FT SECTION OF 30" STEEL PIPE ONTO LOWBOY.AS END LOADER LIFTED THE PIPE BY CHAIN THE PIPE BEGAN TO SWING.EMPLOYEE GRABBED PIPE TO STEADY IT AND STRAINE D BACK.
EMPLOYEE WAS ON CONVEYOR GREASING AND CHECKING ROLLERS. CONTROL PANEL TO CONVEYOR WAS NOT LOCKED OUT EVEN THOUGH SUPERVISOR, ASSISTANT SUPERVISOR AND EMPLOYEES KNEW IT SHOULD BE. CONVEYOR WAS ACCIDENTALLY TURNED ON MOMENTARILY CAUSING EMPLOYEE TO BE ROLLED ON CONVEYOR. HE SUSTAINED SOME MINOR BRUISED AND HAS SOME SORENESS.
