Employee was using a Torque Multiplier. The extension slipped out of the mechanics hand and come back around and struck EE in the left forearm. This caused a laceration in the back side of the forearm.

Employee was greasing D6T Dozer front right lift cylinder when employee's foot slipped off the track. The employee twisted and fell back but caught self on the blade.
Employee was in haul truck when they claimed a boulder shook the cab while being loaded. Causing a back strain.

Employee was operating dozer, backed onto large rock and as started forward track threw rock out letting dozer down hard. This created a hard jolt to employee's back causing a lumbrosacral strain. This employee did not wish to seek medical care until 02-15-06.

EMPLOYEE WAS OPERATING THE LOADER IN THE PIT. AS HE DUG INTO THE PILE, HE HIT A TOE IN ROCK CAUSING THE LOADER TO COME TO AN ABRUPT HALT. THIS CAUSED HIM TO WHIP FORWARD AND THEN BACKWARD IN THE SEAT.
EE HAD BEEN IN PROCESS OF REMOVING ROCK STUCK BETWEEN PULLEY AND BELT. HE WAS RETURNING DOWN CONVEYOR & SLIPPED ON LOOSE ROCK. HE FELL ON CONVEYOR BELT, STRIKING RIGHT FOREARM ON SHARP ROCK, CAUSING LACERATION.

ALLEGEDLY EE WAS PULLING ON PIPE WRENCH TO LOOSEN FILTER ON LOADER WHEN HE FELT PAIN IN HIS LT.HIP/ BACK AREA. THIS RESULTED IN A DIAGNOSIS OF LOW BACK PAIN W/SCIATIC NERVE INVOLVEMENT. THIS INJURY WAS NOT REPORTED UNTIL 8/2/04. IT IS UNDER INVESTIGATION.
Employee was lifting an attachment for the skid loader for welding when he felt a pain in left shoulder. This caused a left shoulder strain with tendonitis.
Ee was hauling materials from south to north side of quarry in haul unit. As he was driving thru tunnel, bed was partially raised causing bed to hit roof of tunnel bringing him to an abrupt halt. As he was wearing his seatbelt this caused his face & shoulder area into the steering wheel causing lacerations to his tongue & lower lip, & cervical contusion.
EE HAD BEEN WELDING ON CRUSHER HAMMER. UPON COMPLETION, HE WAS TESTING THE HAMMER TO MAKE SURE IT MOVED FREELY WHEN IT CAUGHT HIS ANKLE BETWEEN HAMMER AND WALL, CAUSING CONTUSION/ABRASION TO HIS LEFT ANKLE.

EMPLOYEE WAS DIAGNOSED WITH CARPAL TUNNEL SYNDROME, WORK RELATED BY HER REPETITIVE MOTION AS SCALE HOSE ATTENDANT. HER FIRST SURGERY ON HER DOMINANT HAND (RIGHT) WAS USED FOR DATE OF ACCIDENT. SHE WILL HAVE SURGERY ON THE LEFT HAND MARCH 23,2003.

HE HAD JUST STARTED THE GENERATOR AND WALKED AROUND BACK AND SAW A GUARD HAD LOST A BOLT AND WAS FLOPPING. HE WAS MOVING THE GUARD BACK INTO PLACE AND DIDN'T REALIZE THE GUARD WAS BENT AND THE REFORE CAUGHT HIS LEFT HAND IN THE RADIATOR FAN.
HE WAS REPLACING FUSE IN BOX WHEN IT FLASHED & BURNED HIS EYES.
