EE was helping tear down and clean up cables around the plant. EE began experiencing tingling in arms and became sweaty, dizzy and lightheaded. The foreman helped EE into air conditioning and drove EE to the ER at Cedar County Memorial Hospital. EE was tested and observed and dismissed with diagnosis of kidney failure due to dehydration. EE was released.

EE was throwing rocks into the hopper and felt a pop in his left arm. X-rays taken and MRI scheduled but suspect left shoulder strain.

Crew was changing the screens on the secondary lime deck and they had a loader positioned to pull screens from the bucket. Loader bucket bled off and it dropped, causing the railing to fall onto the EE, knocking him into the screen. EE insisted he was fine and not hurt. Next day requested vac day. He worked following day, but called in 9/22/08, wanting medical attention.
EE was using hand held grinding wheel, when 4" grinding wheel broke aprart and struck left knee, causing laceration. A laceration resulted, requiring metal slivers and sutures.
Employee was tightening a bolt. Wrench slipped and middle finger of left hand was struck between wrench and a piece of metal. A laceration/avulsion resulted requiring stitches.

Employee was driving the front end loader and hit a bump and his head struck the right window cutting his head. He was taken to er and a few staples were placed and a cat scan was performed but he was released to return to work. He will return in a week to remove staples.

EE was removing the loader bucket on a 216 cat skidsteer when it fell against his right shin and foot, causing contusion and a chip in the metatarsal and abrasions to his shin.

EE WAS HOLDING A PUNCH WITH CHANNEL LOCKS WHILE COWORKER WAS HITTING THE PUNCH WITH A SLEDGE HAMMER. THEY DO NOT KNOW WHAT HAPPENED, BUT EE HAD BLOOD COMING FROM RIGHT WRIST AREA. X-RAYS REVEALED A SLIVER OF METAL EMBEDDED. ORTH. SPECIALISTS DECIDED NOT TO REMOVE SINCE REMOVAL WOULD CAUSE FURTHER DAMAGE. RESTRICTED DUTY FOR 3 WEEKS.
Employees were re-blocking the cone crusher and the patient caught his fingers between a rairoad tie and the crusher frame. X-rays show no fracture but put on restricted duty until 11-8-04.
EMPLOYEE WAS DRIVING A SMALL FORD TRACTOR WITH A TANK ATTACHED SPRAYING GRAZON ONTO THISTLES. HE COMPLAINED OF NAUSEA AND DIZZINESS. HE WAS TAKEN TO ER AND CHECKED OUT. HE WAS QUICKLY RELEASED AND TOLD TO DRINK LOTS OF WATER AND REPORT TO WORK THE NEXT MORNING. THE DOCTOR DIAGNOSED CHEMICAL EXPOSURE AND FELT HE MAY HAVE HAD AN ALLERGIC REACTION TO SOMETHING IN THE FIELD.
EMPLOYEE WAS CLIMBING BACK INTO HIS BULDOZER AND HIS FOOT SLIPPED. HE GRABBED THE DOOR CAUSING IT TO SLAM SHUT ON HIS RIGHT RING FINGER. HE HAD A LACERATION REQUIRING NINE STITCHES AND IT BROKE THE BONE IN HIS FINGER.

EE WAS CLIMBING OFF HIS DOZER AND SLIPPED AND FELL AND LANDED WITH ALL OF HIS WEIGHT ON HIS RIGHT WRIST. A WRIST BRACE WAS APPLIED AND HE WAS ORDERED OFF WORK DUE TO WRIST STRAIN DISLOCATION.

EMPLOYEE WAS BACKING UP TO BIN TO BE LOADED AND REACHED OUT THE WINDOW TO GRAB THE START/STOP BUTTON. HIS TIRE CAUGHT THE CORD AND JERKED IT OUT OF HIS HAND, BRUISING HIS LEFT THUMB.

EE WAS PULLING ON A CONVEYOR BELT AND FELT A PAIN IN HIS GROIN AREA. REFUSED MEDICAL SURGERY IN FUTURE. EE IS PRESENTLY WORKING UDER RESTRICTIONS.
EE WAS STRIKING DRILL STEEL WITH A HAMMER AND A CHIP OFF THE HAMMER EMBEDDED INTO HIS CHEST. IT WAS REMOVED WITH FLUROSCOPY AND TWO STITCHES CLOSED IT BACK UP.
EE WAS CHANGING WIRE SCREEN CLOTH ON THE PLANT AND IT FELL BACK AGAINST HISS KNEE. IT BEGAN HURTING. HE WENT TO THE EMERGENCY ROOM AND WAS DIAGNOSED WITH PRE-PATELLAR BURSAL INFECTION IN LEFT KNEE WITH CELLUTITIS. HE WAS PUT ON ANTIBIOTICS AND TOLD TO STAY HOME FOR ONE WEEK. RELEASED TO RETURN TO WORK ON 7-30-01.

TRUCK DRIVER WAS UNHOOKING HIS PUP FROM HIS TRUCK BEING ASSISTED BY HIS BROTHER (INJURED PERSON) AND OUR LOADER OPERATOR-DUMP TRUCK BEGAN ROLLING BACK AND PINNED HIM BETWEEN THE TRUCK AND THE LOADER BUCKET-HE MANAGED TO CRAWL OUT ON HIS OWN AND NO INJURIES WERE FOUND OTHER THAN SEVERE BRUISING AND SCRAPED TO THE BACK AND LEFT ARM.
EE WAS USING HAMMER TO DRIVE IN A BOLT AND HIT HIS LEFT THUMB DR COULD NOT TELL THAT IT WAS BROKEN.
