Employee hit shoulder on serge tunnel wall and caused a contusion. Was seen by doctor but release to full duty. On 6-27-2023 employee was seen by doctor and given a shot. Making it a recordable released to full duty again.
Employee was walking under a piggy back conveyor and struck the lower support. The employee was wearing a hardhat.

Employee states ee was loading a broken gearbox into loader bucket so it could be loaded into the back of a pickup to be transported to shop for repairs when ee felt a burning pain in right bicep. Employee went to the doctor on 9/18/18 and was placed on restricted duty making this a reportable injury.

The employee did not report incident to us until 3/15/16. EE was picking a tail pulley guard up and strained EE's back.

Employee was looking up changing a butterfly assembly and something went into eye. Became reportable on 7/29/15 when employee went to the doctor and was diagnosed with an eye infection and prescribed eye drops. No foreign body was present.

Employee does not recall any specific thing that caused his back pain but on or around the noted date he noticed his back was hurting and told another co worker.

I was cutting on the bottom of a washer on the Dredge, when the cutting torch popped and sparks flew from the metal & rust that I was cutting. At the time I felt something stinging in my eye. Tried to wash it out with no success, then reported it to my Supervisor. I was wearing safety cutting glasses at the time.
Coming down the stairs at stilt shed, lost footing fell down and twisted ankle.

Employee got out of his truck and was walking toward ramp to dock barge. He slipped on ice and fell. This resulted in a low back strain.
Employee was stepping out of tunnel at a slight angle. Left leg slid out as if doing the splits. He reports feeling discomfort and pain at that time, however, he did not go to the clinic until 12/4/08. therefore, this claim was not reportable until 12/4/08. He has a strain to the left knee.
Employee was walking on a concrete slab, when he accidently stepped off the side, which caused his foot to roll sideways and caused an ankle sprain.
Employee was climbing out of pump well when he slipped and fell, hitting a piece of iron, causing a cut to his left shin. This injury is only a scrap, however, the clinic put him on modified duty because of his job.
This accident has just become reportable as employee did not turn in accident until May 18,2008. He stated he fell on ice and landed on his left shoulder. It continues to bother him and the clinic put him on modified duty.

Employee was working at the slurry. He tripped on the walkway and fell. He tried to catch himself which resulted in a fractured right thumb.
Employee was walking up a slope by a conveyor when he slipped on some rock and strained his left knee.

Employee was taking blower off of air compressor when piece fell on his thumb. This caused a cut that required stitches in his left thumb.
Employee was beating ice off of conveyor with a hammer and this caused a cervical strain.
2005 · 6 incidents
Employees were moving a cutting edge from the bucket of a front end loader. The corner piece ws being placed and the employee's left index finger was in the way and was mashed. This required 7 stitches.
Employee had finished fueling the boat and was dragging the fuel hose up the bank he slipped and fell cutting his right knee. The laceration required nine stitches.
Employee was helping truck driver get sand off of his tailgate. The truck driver closed tailgate on employee's left hand. Employee had to have stitches in his left hand.
EE slipped and fell but did not want to visit the clinic. On 4/12/05 EE decided to visit the medical clinic. EE is on full duty, however, the doctor wants EE to have therapy on hip, therefore this accident is now reportable.
Employee was pulling wires across the deck of the tow boat. A wire was kinked and while trying to get the kink out employee twisted and back popped. EE has a low back strain with spasms.
Employee was exiting his truck and slipped on ice and fell. He thought he would be alright and did not turn in a claim until 1/20/05. His back was still hurting so he went to an occupational clinic. He has a low back strain.

Employee was welding and received flash burns to both eyes. He was wearing protective equipment. A reflection from the water caused the flash burns.
Employee was cleaning sand screen with a brush. The brush got caught, and the employee jerked on the brush, it came back and hit him in the mouth. His front tooth was broken.
Two employees were arguing when one pushed the other and one fell injuring his shoulder.
Accident was reported to us on 4/29/04. Employee was installing a tilt cylinder. He slipped and fell landing on the highloader platform. He strained his lower back on the right side.
EE was pulling on wire rope and felt pain in groin.

EMPLOYEE WAS CHANGING A BARGE. THE ROPE TIGHTENED AND THE GAVEL BROKE. IT HIT EMPLOYEE ON THE SHIN OF HIS LEFT LEG CAUSING A LACERATION THAT REQUIRED 7 STITCHES.

SUBCONTRACTOR HIRED TO HOIST(PICK) EQUIPMENT. WHILE REELING CABLE THE OILER SLIPPED AND HIS HANDWAS PULLED INTO THE SIDE OF THE DRUM.

EMPLOYEE WAS RUNNING HIGH LOADER AND STARTED HAVING PAIN IN THE UPPER, MIDDLE, AND LOWER BACK.
EMPLOYEE FELL WALKING DOWN THE SIDE OF THE BARGEBRUISING HIS TAILBONE.
EMPLOYEE WAS WALKING ON THE ROCK BARGE. HE STEPPED ON A MACHINE COVER AND IT FELL IN AND EMPLOYEE FELL INTO THE HOLE.
EMPLOYEE BUMPED ELBOW. DIAGNOSIS WAS BURSITIS. HE HAS DONE REGULAR DUTY. ON 10/27/00 SURGERY ISBEING ADVISED.
EE WAS RUNNING WITH A CHAIN AT THE SLURRY. HE HIT HIS HEAD ON THE STACKER CONVEYOR. HE CUT HIS HEAD AND IT REQUIRED 8 STITCHES.
EMPLOYEE WAS PULLING ON A HEAD WIRE LOCATED ON THE DREDGE. HE STRAINED HIS LOWER BACK.
EE SLIPPED ON THE ICE AND FELL. IN THE COURSE OF FALLING HE TRIED TO CATCH HIMSLF AND HIS ARM HIT A BARRELL STRAINING HIS SHOULDER.
EE WAS CHIPPING FROZEN SAND OFF OF A CONVEYOR. SAND FLEW IN HIS EYE AND CAUSED AN ABRASION.
EE WAS WELDING AND HE STRUCK HIS FINGER WITH A HOT WELDING ROD.

EE WAS ATTEMPTING TO ROLL WELDING LEADS THAT WERE STRETCHED TO THE BARGE AND THEY GOT HUNG UP ON THE ROCKS. EE WENT TO RELEASE WIRES FROM THE ROCKS. HE SLIPPED ON THE ROCKS AND HIS RIGHT FOOT FELL INTO A HOLE. AT THE TIME RIGHT FOOT FELL INTO A HOLE. AT THE TIME RIGHT FOOT FELL INTO THE HOLE, HIS KNEE POPPED.
MOTOR HIT LOG AND RAISED UP IN THE LOCKING POSITION HAD TO REACH DOWN TO UNLATCH THE MOTOR,THE MOTOR CAME DOWN BETWEEN THE STERN OF THE BOAT AND THE MOTOR HIS FINGER WAS ON THE RELEASE LEVER.W
