Employee was operating the dragline when the wedge came out of the dump cable socket. As the employee began to reinsert the cable back into the socket with the help of another employee, the short end of the cable popped out of the socket and hit EE in the chin. Received 4 stitches to the chin.

While replacing a tail pulley on the lignite conveyor, employee tried to move a gear box out of the way and felt a pull/pain in his lower back. Went to Wixom Occupational Health Center on 5/12/06 where they distributed 2 bottles of pills to him. Naproxen (500 mg) 14 pills and Skelaxin (800 mg) 12 pills.

EE and co-worker were removing teeth from the drag bucket. Injured had a large pin hammer and co-worker had a sledge hammer, hitting the pin hammer. Pieces of steel from the tooth on the bucket chipped off and hit ee in the calf of his left leg. He ended up with a piece of steel lodged in his left calf.

EE STARTED A ROAD GRADER TO LOAD IT ON A LOW BOY& DISTURBED A BEE'S NEST IN THE DOOR RESULTING IN 3 BEE STINGS. HE HAD AN ALLERGIC REACTION & WAS GIVEN PRESCRIPTIONS & RELEASED WITH NO RESTRIC TIONS. CLINIC INFORMATION WAS NOT RECEIVED UNTIL 1/15/03.
