BURROUGHS MATERIALS CORP · Over-exertion in lifting objects

Employee was moving 4'x8' screens on tower 1 with another employee. They lifted the screens about 2 inches off the ground and approximately 10ft down the catwalk. Shortly after this process was completed the employee felt a strain in EE's stomach and noticed a protrusion from EE's belly button. Umbilical hernia.

Employee arrived at work around 7am and parked EE's vehicle in the employee parking lot near a loader that EE was going to use for the day. EE exited EE's vehicle and walked over to the loader to check the fluid and start it. As EE was walking around the loader EE slipped and fell on some ice that was hidden by some fresh snow.

Employee was using a 5ft long steel bar to unplug a chute with 8"x18" material on the RIPRAP trammel when ee pinched finger between the bar and a steel plate causing a laceration.

Employee was standing on the platform next to his entry door on 980M Cat Loader to clean the 45 degree window off when his foot slipped off the platform; he was holding onto the mirror as this happened so he caught himself with his right arm on the mirror causing a strain to his bicep.

Employee was using an air compressor through a 2" line to clean out under a tail pulley and as he went to set the line down the ball valve turned on which caused the line to flair around and strike the lineman in the head causing a 2.5" cut required 4 staples to close.

While checking the top deck of the grizzly, employee tried to move a stone that was lying on top of the grizzly bars. He heard and felt a tear in his left biceps. Employee was rechecked on 07/08/2013 and put on restrictive duty.

Employee was repairing screen, as he lifted the balance wheel sheave with a 1 1/2 ton coffin hoist the nut on 1/2" eyebolt backed off, this allowed the balance wheel sheave to fall hitting employee on both legs below the knee, landing on the deck, and falling over onto right ankle. Contusion to both legs and right ankle.

Ice was covered with snow and employee didn't see ice. Employee slipped on ice injuried right arm & shoulder. Employee was hurt on 12/11/09 and reported incident to management that day, at this time employee did not feel he needed to see doctor. On 1/4/10 reported pain in his shoulder, was sent to clinic and was put on restricted duty along with prescription medicine.

Employee alledges while working on the north side of #3 tower, loosening bolts on the screen deck and pulling a wrench toward his body, he felt a pop in his right shoulder. The pain continued for the remainder of the day, increasing through the night. He went to the clinic the next day where he was given medication to take every 4 - 6 hours as well as restricted work duties.

Lid of storage box on loader fell onto employee's right hand ring finger, lacerating skin on right side of fingernail. Originally did not think this was reportable since there was no lost time and restrictions did not inhibit employee from doing his regular job. Now it has been determined that the rx given makes this a reportable incident since it was more than a single dose.

Employee was lying on the ground underneath loader trying to tighten bolts on center pin using a ratchet and breaker bar. The bolt snapped off and the employee jerked himself out of the way from the falling cheater bar. At this time he felt some pain in his shoulder and on up into his neck. Pain increased as the day progressed.
