Employee was getting out of the 982-M5 front end loader to inspect the surge stacker, which had stalled. They were climbing down the ladder and were on the second step from the top when they felt discomfort in their right leg.

An employee was changing out a bad motor on the 8 X 24 screen at the wash plant. EE removed the mounting bolts and then was using a prybar on the base of the motor to rotate it clockwise. While prying in an upward motion from the base of the motor the prybar slipped and EE's momentum caused EE's right hand to strike the electrical box on the side of the motor.

Employee was grabbing a 5-gallon bucket of oil off the spill containment in the lube shed and placing it into the back of the pit truck when their right middle finger got wedged between the bucket and the handle injuring their finger.

Employee was climbing down the stairs of the Crane cab when the cab door unexpectedly closed due to windy weather and caused employee's lunch box to fall from employee's hands and down onto the floor of the crane near the hoist drums. The employee climbed down to the floor of the crane in between the hoist cables to pick up the lunch box and rolled left ankle.

Employee was operating loader and backed away from backside of feed hopper, with bucket in a raised position. Traveled approx. 28 feet away from hopper with the bucket raised and backed up on unleveled ground causing loader to flip on it's side.

Employee was walking onto Dredge from floating conveyor, he stepped onto Dredge and stepped into man hole for pontoon. This cover was removed by trespasser over the weekend. Employee fell onto his left side and lower ribs, contusion to left ribs.

Michigan State Police came to the Holly Sand & Gravel site because a missing person and possible suicide had been reported. At 11:20 AM they found a body on Holly Sand & Gravel site. This person was not an employee of Holly Sand & Gravel. The deceased person had died for a self inflicted gun shot wound to the head.

Employee was putting a wiper on grizzly conveyor when it slipped out of cradle hitting him on the chin and then hit ladder, knocking him off. The ladder hit him below the knees on both legs. Originally this was a first aid only. Employee's second check up with doctor (10/22/07) revealed swealing of his left knee at which time he was put on restricted duty.

EMPLOYEE COMPLAINED OF PAIN RADIATING FROM HIS GROIN DOWN HIS LEG AFTER OPERATING A SCRAPER.
Employee was operating a scraper when he ran the scraper over uneven ground causing him to be jostled. Employee was diagnosed with a fractured lumbar spine. Subsequent investigation revealed that cushion hitch had not been activated.

Employee was repairing a splice on the field conveyor. While cutting out a patch to fill the hole, the utility knife passed through the belt (patch material) and struck right index finger. Finger was lacerated requiring 5 sutures and a tetanus shot.
While walking up hill in soft sand, employee's left knee buckled. Clinic gave employee a RX for #21, 800 mg Ibuprofen tablets as well as other restrictions that are not necessarily inhibiting him from doing his normal job.

While wiring up a motor, employee picked up a piece of wire and his safety glasses dropped down on his nose and the wire then hit his left eye.
While lifting gate weights for placement onto edge of screen, employee felt a pull in his lower back.

WHILE WORKING ON A BEARING, A PIECE OF STEEL HIT THE EE'S FOREHEAD JUST ABOVE HIS SAFETY GLASSES. EE DIDN'T REALIZE THAT HE HAD ANYTHING UNDER THE SKIN UNTIL 7/25/03 AT WHICH TIME HE WENT TO THE CLINIC AND THEY REMOVED THE FOREIGN BODY. NO DAYS LOST AND NO RESTRICTED DUTY.
SCREEN PANEL FELL OUT OF ANOTHER EE'S HAND WHILE INJURED EE WAS PASSING IT UP TO HIM ON THE NEXT LEVEL AND IT CUT THE END OF HIS NOSE AND PUFFED UP HIS LIP. HIS NOSE REQUIRED SUTURES THAT ARE TO BE REMOVED IN 7 DAYS.

WHILE SHOVELING OUT FINE SAND RECOVERY TANK BELOW WATER LEVEL, EE STRAINED HIS RT. SHOULDER. INCIDENT WAS NOT REPORTED TO SUPERVISOR UNTIL 10/12/02. EE WENT TO HEALTH CENTER ON 10/14/02, IN WHICH HE WAS PUT ON RESTRICTED DUTY.
EE DIDN'T KNOW OF ANY ONE INSTANCE THAT MAY HAVE RESULTED IN THIS INJURY, HE CLAIMS THAT HIS LOWER BACK JUST GOT SORE & FELT STRAINED ON THIS PARTICULAR DAY. THIS INJURY WAS NOT WRITTEN UP UNT IL 9/25/02. THE EE DID NOT SIGN ACCIDENT REPORT UNTIL 9/30/02. THE ACCIDENT REPORT ARRIVED AT MY OFFICE ON 10/3/02. STILL WAITING FOR MED. REPORTS FOR EE. HE WAS PUT ON RESTRICTED DUTY.
EE WAS USING A 5' PRY BAR TO FREE AN OLD LINER FROM A 4 1/2' SHORT HEAD MANTLE. THE PRY BAR SLIPPED & THE EE LOST HIS BALANCE. WHILE STEPPING BACKWARDS TO CATCH HIS BALANCE, HIS LEG TURNED & A S THIS HAPPENED HE FELT A SNAP IN HIS LOWER LEFTLEG. AT THAT TIME, HE FELL TO THE GROUND. SURGERY WAS REQUIRED BUT NO MEDICAL REPORTS HAVE BEEN RECEIVED AT THIS TIME.

USING HIS RIGHT HAND, EMPLOYEE WAS CHECKING AN ALTERNATOR. HIS LITTLE FINGER WAS STRUCK BY THE COOLING FAN BLADE.
WHILE UNLOADING TROUGHING IDLERS OFF THE BOOM TRUCK DECK FOR INSTALLATION ON THE ROHR DREGE #1 CONVEYOR, EE SLIPPED ON THE 1 INCH OF FRESH SNOW AND FELL TO THE GROUND FROM GROUND LEVEL. THIS C AUSED THE IDLER HE WAS CARRYING TO STRIKE HIS RIGHT ARM AGAINST THE GROUND.
EE WAS ACTING AS HOT SAND PLANT OPERATOR. ON THIS PARTICULAR DAY, ALL AREA SCHOOLS WERE CLOSED DUE TO ICY CONDITIONS. THE AREA AROUND THE ELECTRICAL CONTROLS, FEEDER AND WHERE HIS VEHICLE WAS PARKED HAD BEEN SANDED. DURING THE SHUT DOWN PROCESS, EE WENT TO SHUT OFF THE PROPANE CONTROL AND FELL ON HIS LEFT SIDE SINCE THE AREA WAS NOT SUFFICIENTLY SANDED.
