UNKNOWN AT THIS TIME: EMPLOYEE WAS FOUND WITH HIS RIGHT ARM CAUGHT BETWEEN THE HEAD PULLEY AND CONVEYOR BELT WITH THE PULLEY STILL RUNNING. A FELLOW EMPLOYEE HEARD HIS CRY FOR HELP, CUT THE P OWER OFF, HE THEN FELLT OT HE GUARD BELOW, THAT WAS ON ANOTHER CONVEYOR TAIL PULLEY.

EMPLOYEE WAS HELPING TO LOAD A SHOT FOR BLASTING. HE HAD BEEN CARRYING 50# SACKS OF ANFO ON HIS SHOULDER AND HIP TO THE BLAST SITE FROM DELIVERY TRUCK. INJURY OCCURRED 11-29-94 BUT EMPLOYEE DI D NOT REQUEST TO SEE DR. UNTIL 12-5-94.

EMPLOYEE WAS ATTEMPTING TO R4EMOVE SPILLAGE (LOOSE ROCK) FROM CATWALK WITH THE INSTEP OF HIS RIGHT FOOT. (KICKING MATERIAL FROM CATWALK). WHEN HIS FOOT CONTACTED THE LOOSE ROCK HIS RIGHT KNE E PIPPED ON THE INSIDE AND PAIN CAME TO HIS RIGHT KNEE. SHOULD HAVE USED A SHOVEL.
