Employee was removing a loader fork bar. When he unhooked a 2" clevis he was using to pull the bar out the clevis swung down and pinched his left middle finger. This caused a bruise under the fingernail.

Employee was splicing a 995 volt cable. The utility knife he was using slipped and lacerated the end of his left little finger. The laceration required one stitch.

While operating a 988F loader employe's left eye became irritated. This irritation became so bad that employee called for assistance.
Employee came to work with Dr's excuse & a prescription for Cecebrex & Vikaden. Foreman told employee he could not operate equipment while on Vikaden. Employee stated that he went to Dr. for pain in his shoulder, wrist and right foot. He also stated tha his right foot was still giving him problems. The Dr. wrote to not use right foot on pedal that Miner Operator has to use.
WHILE PULLING OUT OF #18 HOLE (WEST PIT, X-SEAM), EMPLOYEE WAS PULLING THE CAR PIN. THE PIN BEING STUCK OR IN A BIND, THE EMPLOYEE MOTIONED FOR THE LOADER OPERATOR TO BUMP THE CAR. THE EMPLOYEE'S LEFT HAND WAS ON THE PIN WHILE LOADER MAN BUMPED THE CAR CAUSING HIS RIGHT THUMB TO BE MASHED BETWEEN PIN HANDLE AND THE CAR FRAME.
While shoveling coal from catwalk on left side of launch pad, ee said after 2nd shovel of coal he felt 2 pops in lower back.
