Man was pulling fabric cloth up sides of ditch so he could add more rock in ditch. He was having problem staying on sides of ditch so excavator operator moved bucket over for him to place foot against while he was pulling cloth but foot slipped inder cloth and bucket hit his foot.

EE WAS USING A TORCH. THE BURNING BAR BLEW OUT OF THE HANDLE AND THE FLAME BURNED HIS LEFT WRIST.

WHILE REMOVING A LILLY CONTROLLER FROM THE HOIST HOUSE, WHEN THE LAST BOLT WAS REMOVED, THE LILLY CONTOLLER FELL STRIKING HIM ON THE FOREHEAD. HE RECEIVED TWO LACERATIONS TO THE FOREHEAD. ONE REQUIRED 4 SUTURES AND ONE REQUIRED 6 SUTURES. A PART OF THE LILLY CONTROLLER WAS WHAT FELL. IT IS A GEAR MOUNT PLATE.
