The operator was notified of a claim for Occupational Pneumoconiosis after mining has ceased and the affected employee was laid off. The operator wishes to report the Illness Claim and terminate the affected miners employment since mining equipment has been removed and mine entrances are sealed.

After reviewing part 50 reporting and present status of the sealed mine we wish to report the illness claim and terminate employment on date 6/20/16 that mine was reported abandoned to MSHA.

Employee was spot bolting 10 foot cable bolts. One piece of steel was in the hole when he was pushing a piece of steel up to connect it together he got his middle finger between the steel and the top cutting the end of his finger off.

Two employees were replacing bucket on a 488 scoop. Injured employee stated that he was having trouble lining up the hinge pin in the bucket hinge and was using bare finger to find out why there was misalignment. The bucket shifted causing the fingertip to be pinched between the hinges
