TIGHTENING THE HEAD DOWN A CRUSHER WITH CABLE AND LOADER. THE PROBLEM IS THAT A CHAIN INSTEAD OF CLEVIS WAS USED TO FASTEN CABLE TO PAYLOADER BUCKET. THE HEAD STOPPED, CHAIN BROKE AND THE CABL E FLEW AND CAUGHT HIM WHERE HE WAS STANDING WAS NOT A SAFE LOCATION.

EMPLOYEE WAS THROWING LARGE METAL BRACKETS AND CRUSHER TOOLS ON FLOOR OF STORAGE TRAILER WHEN A PIECE OF SCRAP METAL WAS CONTACTED MAKING IT COME UP AND STRICKING EMPLOYEE ON THE SIDE OF HEAD, EMPLOYEE RECEIVED A BRUISE ON THE SIDE OF HEAD.
