Mechanic was working on Kawasaki Loader in shop. The drive shaft swung and pinched employees hand between the U Joint and the mainframe of the loader, cutting the webbing between my thumb and first finger. He received 6 stitches. Justifying a reportable medical injury with no lost days.

Employees were changing cutting edges on WA500 Komatsu.They were loosening bolts with a 1 inch air gun. The bolts started turning all the way. The injured EE went to push the bolt in place with his hand & other employee hit the gun & the bolt started spinning & cut his index finger on his left hand. Doctor applied Stitches to finger.

EE WAS OPERATING THE PLANT JAW CRUSHER. HE HAD AJAM AFTER HAMMERING ON THE JAM WITHOUT SUCCESS HE WALKED OUT ON THE PLATFORM TO LOOK IN THE CRUSHER. WHEN HE WENT BACK IN JAW HOUSE, HE FELT SOM ETHING IN HIS EYE. HE TRIED WASHING IT OUT, BUT DID NOT HELP. HE THE CALLED FOREMAN TO REPORT ACCIDENT

INDEX FINGER ON HIS RIGHT HAND WAS CUT WHILE PLACING TAIL PULLEY GUARD ON A CONVEYOR. HIS FINGER WAS CUT AND REQUIRED 4 STITCHES. THE INJURY OCCURRED WHEN HIS FINGER WAS TRAPPED BETWEEN CONVEY OR FRAME AND GUARD. THE EMPLOYEE WAS WEARING GLOVES WHEN THE ACCIDENT HAPPENED.
WHILE GETTING A HAMMER OUT OF THE TOOL BOX ON THE BACK OF THE LOADER, THE LID FELL CLOSED ONTO HIS SMALL FINGER ON HIS RIGHT HAND. HIS FINGER REQUIRED 4 STITCHES. THE EMPLOYEE WAS WEARING WORK GLOVES WHEN THE ACCIDENT HAPPENED.

EE WAS SERVICING THE PIT LOADER AND CHANGING A O-RING, AND AFTER DOING THIS HE RETURNED A WRENCH THAT HE HAD BORROWED FROM A CONTRACT WELDER. ON HI SWAY BACK TO THE LOADER THE EE SLIPPED ON A ROCK AND FELL DOWN HURTING HIS WRIST. HE DID NOTGO TO THE DR AND HE HAD A SPRANG LEFT WRIST.
THE PIT TRUCK DRIVER WAS BEING LOADED WHEN A ROCK FELL OFF THE CORNER OF THE PIT LOADER BUCKET AND CAME INSIDE THE CAB OF THE TRUCK AND STRUCK THE EMPLOYEE ON THE TOP PART OF THE LEFT HAND.

ON 2-15-89 AT 300 PM IN THE AFTERNOON THE EMPLOYEE WAS ATTEMPTING TO PRIME PLANT PUMP IN THE PRCOCESS OF DOING THIS HE SLIPPED AND FELL CUTTING HIS SHIN ON THE LEFT LEG. HE CAME STRAIGHT TO TH E OFFICE AND REPORTED THE ACCIDENT CUT ON SHIN OF LEFT LEG.
