Employee was stepping down from loading dock onto boat. When he stepped down his right foot landed on a pipe laying on the deck of the boat, the pipe rolled twisting his foot.

Employee was cutting vertical pipes off of gate at mine enterance to install winter curtains. When employee went to move to the next cut he inadvertantly placed his hand on the area he had just cut causing burns to his right palm and fingers.
Employee was using a cutting torch to cut phase arm bracket bolts. The employee had to lay on his side to access the bolts. While cutting, a piece of slag fell into the employee's right ear. The employee reported the incident on 11/22 but did not seek medical treatment until 11/27.

Bin chute had plugged with primary crusher discharge (minus 5" limestone). While men at top of bin were rempoving rock, personnel were at bin discharge removing rock. A rock fell from top of bin and landed on left hand of person while hand was positioned on angle iron hand rail. Reporting delayed waiting on medical determination of condition.
A large roof fall occured in DD @ 11 intersection, luckily the scaler or any other personnel were not in the area at the time. The operator had intentions of scaling a large rock when the machine broke down on the 27th.

Operator was attempting to remove a piece of broken drill steel from the striker coupling by impacting the striker with a 2lb. steel hammer. A sliver of the striker broke off and struck the eployee in the left arm bicep, imbedding in the arm.
Employee was striking a steel wedge with a 4lb. hammer and the employee missed the steel wedge and hit his left thumb which was holding the wedge in position.

ELECTRIC POWER FAILURE CAUSED LIGHTS TO GO OUT AT CRUSHER CONTROL CENTER. CRUSHER OPERATOR WAS RESETTING BREAKER FOR ELECTRICAL POWER AND STEPPED OFF OF CATWALK AND SPRAINED LEFT ANKLE.
BLASTER PARKED PICKUP AT T@-1 TO TAKE WATER TRUCK TO WATER ROADS IN MINE. AS HE WAS MAKING HIS ROUND HE SAW AN ORANGE GLOW WHERE HE PARKED THE PICKUP TRUCK. HE DROVE TO THE TRUCK AND IT WAS ON FIRE. HE USED THE FIRE EXTINGUISHER FROM THE WATER TRUCK BUT COULD NOT EXTINGUISH THE FIRE.
