Using a forklift, a large piece of steel plate was being flipped. The plate flipped opposite the way it was intended. When it fell, it hit the injured on the back of the heel and broke his lower leg.

WELDER WERE LOWERING A TRELEX RUBBER PIPE WITH A CAST IRON ELBOW ATTACHED. THE LOWERING CHAIN GOT CAUGHT ON THE FLOORING ABOVE THAT CAUSED SLACK IN CHAIN. IT BROKE LOOSE FROM THE FLOOR AND TRE LEX HIT EE ON THE LEG AND KNOCKEDHIM OFF THE PLATFORM. HE FELL 6 TO 7 FT TO THE FLOOR.
