THE VICTIM RECEIVED FATAL CRUSHING INJURIES WHILE WORKING UNDER THE BOOM OF A BANTAM 350 CRANE. THE VICTIM AND CRANE OPERATOR WERE IN THE PROCESS OF REMOVING THE BOOM FROM THE MACHINE. THE VIC TIM APPARENTLY POSITIONED HIMSELF UNDER THE BOOMAND KNOCKED OUT THE 2 BOTTOM PINS CAUSING BOOM TO FALL.
HE CUT WATER OFF BEHIND ROTARY BREAKER, STARTED BACK DOWN STEPS, SLIPPED AND FELL DOWN TWO OR THREE STEPS AND INJURED LOWER BACK AND RIGHT KNEE. HE LATER WENT TO THE DOCTOR ON 9/9/03 WHO STARTED PHYSICAL THERAPY (ONE MONTH). STARTED MISSING ON 9/9/03.

THE EE STATED THAT HE WAS USING RED DEVILS TO LOWER AN OLD IMPACT BOX DOWN FROM CHANNEL. WHILE USING RED DEVILS THE HANDLE FLEW UPWARD AND STRUCK HIS LEFT JAW.

AFTER FELLOW EE BECAME TRAPPED UNDER CRANE BOOM EE JUMPED FROM CRANE AND ATTEMPTED TO LIFT BROOM BY AHDN. AFTER BEING UNABLE TO LIFT BOOM, EE STARTED RUNNING TOWARD PICK UP TRUCK TO GET LIFTIN G JACK WHEN HE FELL AND CAUGHT HIMSELF WITH HIS RIGHT ARM. EE WAS IN A STATE OF PANIC.
2 EMPLOYEES WER9 CARRYING H-BEAM FROM BACK OF DUMP TO TOP OF DUMP WHEN HE FELT PAIN IN LOWER BACK. EMPLOYEE WORKED 4 DAYS FOLLOWING ACCIDENT, WAS OFF 1-12,1-13 THEN RETURNED TO WORK.

THE INJURED EE WAS LIFTING CRUSHER PAD TO REMOVE CHAIN FROM UNDERNEATH PAD & FELT PAIN IN HIS GROIN. AT FIRST THE ACCIDENT WAS A FIRST AID CASE THEN THE EE HAD TO HAVE SURGERY FOR A RUPTUR E ON 3-30-83 WHICH NOW IS MEDICAL CASE.
