The dipper stick portion of the hydraulic hammer assembly was cribbed on a pallet. Employee stepped on the edge of the pallet which shook the stick and hit the outside of EE's foot. Employee said was okay and continued working. When EE went home and removed the boot, EE saw the blood and laceration and went to the hospital for treatment where EE received 10 stitches.

Employee was standing on a 4' step ladder working on a grease real. EE was lining up a bolt and while EE was reaching, the ladder slipped and EE lounged forward hitting EE's face. Employee went to the Medicus Urgent Care, Dunmore where EE received (4) stitches in EE's lip and (3) stitches above EE's left eye.

Employee was heating metal with a propane torch at Primary #1 hopper. The flame went out and EE went to relight the torch. The gas built up and exploded causing burns to face, hands and chest. EE's clothing was on fire, EE laid and rolled on the ground to extinguish fire. Employee was taken to ER for medical treatment.

Employees were working on the J50 Portable Jaw Crusher. The plant was down and employee was helping the mechanics repair the radiator. Employee was standing on the hydraulic tank handing the mechanics the parts. Employee then walked onto a ledge (6-8") to get down, lost footing and fell approx. 10-11 feet.

Employee was bent over cutting a piece of conveyor belting on the screen deck and while trying to cut though the rubber, EE cut EE's right leg (calf). EE received first aid and went to the Medicus Urgent Care where EE received three stitches.

Employee was walking to weld shop and the water truck was parked next to door. He walked behind the water truck and a metal bar was sticking out. He ducked and thought he cleared the bar but he didn't and he hit his head on the bar.

Employee was using a razor knife to cut wire and his hand slipped and he cut his finger on left hand.

Employee was constructing a skid base that was elevated approximately 3' off the ground. The forklift operator was putting a beam on deck when the strap slipped causing the beam to move. Employee jumped off skid to avoid swinging beam and suffered injuries as a result of the fall.

EE was changing the cutting edge on the JD844. While holding the edge with one hand and removing the bolt with the other, the bolt spun. It cut through his glove and cut his finger. First aid was admin. and EE was taken to the hospital for care.

EE was changing cutting edges. The cutting edge were sitting on the jack and he had his hands liner the edge lining up the holes. Another EE came to help and moved the jack. The edge was wedged against the bucket. The edge released pinching ee left hand.

EE was struck in the head by a bar he was using to remove a cylinder pin. When the pin came out, the shear jaw dropped about 2 inches which caused the bar to swing and hit EE's head. EE was taken to CMC Hospital where he required 6 stitches above his right eye.

EE was climbing up onto his D8L dozer. He slipped attempting to pull himself up the first step onto the track pads to reach the second step. He fell backwards first landing on his feet, then hitting his tailbone & then his back on the ground. Weather conditions were good and temperature was approx 41 degrees. He was covered with a blanket and was transported to a local hospital.

He was manually sliding a steel plate off a wooden pallet so he could hook a clamp to it. The plate slide quick and pinched his finger against a wheel on the welding machine next to him.

The vendor was backing up and his foot slipped off the brake and hit the gas pedal causing the van to accelerate and strike the employee.

He was grinding a pulley wheel. His hand slipped and the grinder wheel struck his left hand middle finger.

Attempting to move a portable litter fence. He stopped forklift with the forks under side of the fence & exited the forklift. With forklift running, he went between the cab & lifting mechanism of the forks to chain the fence to the fork cage. He stepped backwards against the control lever & mechanism moved backward, pinning him between cage & the roof of the cab.

HE WAS WINDING THE FUEL HOSE AND IT WAS TWISTED. WHEN HE HIT THE BUTTON THE NOZZLE POPPED OFF AND HIT HIM IN THE RIGHT SIDE OF THE FACE.

EMPLOYEE WAS CHIPPING SLAG OFF WELDING AND HE STRUCK HIS FINGER.

EE WAS REPLACING THE HEAD OF A 12 LB SLEDGE HAMMER WHEN EE STRUCK THE PINKY FINGER ON RIGHT HAND

EE WAS CLEANING MUD FROM THE MODIFIED STACKER OF PRIMARY #1. THE EQUIPMENT WAS MUDDY FROM THE PREVIOUS NIGHT'S STORM. EE SLIPPED ON WET MUD ON PLATFORM AND TRIED TO JUMP OFF (4') TO PREVENT A FALL. WHEN EE LANDED ON HIS FEET HE ROLLED OVER ON HIS RIGHT ANKLE, CAUSING PAIN.

Employee was moving in and around a litter fence and tripped over a section of the fence. he fell backwards onto the piping. An ambulance was called where he was taken to the hospital and treated for three broken ribs and a collapsed lung.

HE WAS PERFORMING GENERAL MAINTENANCE AT PRIMARY #1. THE WIND BLEW OUT THE FLAME OF THE TORCH HE WAS USING. THE GAS BUILT UP AND WHEN HE WENT TO STRIKE IT AGAIN THE PROPANE BLEW UP IN HIS FACE .

HE WAS CLEANING OFF A BELT SCRAPPER ON A HEAD WHEEL WHEN THE BAR SLIPPED THROUGH PAST THE SCRAPPER AND WAS CAUGHT BETWEEN THE BELT HEAD WHEEL & PULLEY. THIS PULLED HIS GLOVED HAND INTO THE BEL T. THE BELT WAS MOVING AT AN IDLE SPEED WHEN THE INCIDENT OCCURRED.

TRIMMING INSULATION OFF HEAVY ELECTRICAL WIRE IN 10 DEGREE WEATHER AND THE RAZOR KNIFE SLIPPED AND HE CUT HIS THUMB AT THE SECONDARY #1.

INSTALLING A BUSHING IN THE MAIN BOOM OF THE WARE HAMMER AND THE BOOM SHIFTED OFF THE CRIBBING. IT FELL AND HIT HIM IN THE HEAD.

HE WAS REASSIGNED TO ANOTHER TRUCK, SO HE BROUGHT HIS TRUCK IN AND WHEN HE REACHED BEHIND THE SEAT TO CLEAN OUT THE TRASH HE HURT HIS BACK.

WHILE INSTALLING A WATER VALVE AT PRIMARY #1 HE WAS ADJUSTING HIS SAFETY BELT AND HIS HAND SLIPPED AND HE HIT HIS TORCH.

WHILE CHECKING OIL ON START-UP PROCEDURE, HE WASSTEPPING FROM ONE TRUCK TO THE NEXT AND HE SLIP-PED AND FELL APPROX. 10 FT.

HE WAS INSTALLING A SCREENDECK AT SECONDARY #1. WHILE TIGHTENING IT DOWN THE SOCKET SLIPPED AN DHE HIT HIS THUMB. HE REFUSED TREATMENT AT THE TIME. HIS THUMB WAS HURTING AND SWOLLEN IN THE EVE NING AND HE WENT TO THE HOSPITAL INT HE MORNING 7-25-01 TO GET IT CHECKED.

AN EMPLOYEE WAS RIGGING A BUNDLE OF ANGLE IRON PIECES ON THE BACK OF A FLAT BED TRAILER. AS THE ANGLES WERE PICKED, THEY WERE NOT RIGGED EVENLY. WHEN THE OPERATOR SET THE PIECES BACK DOWN ON T HE TRAILER, THE RACK SUPPORTS THEY WERE SETTING ON BECAME UNSTABLE AND FELL TAKING THE RIGGER WITH.

HE WAS INSTALLING A CUTTING EDGE ON THE JD LOADER #4. THE BUCKET WAS 18" HIGH FROM THE GROUND WHEN THE CUTTING EDGE FELL ONHIM. HE DID NOT REPORT THE ACCIDENT UNTIL 1300 BECAUSE HIS RIGHT FOOT WAS HURTING HIM.

HE WAS IN THE PORCESS OF CHANGING THE BIT ON THE DRILL. HE WAS WALKING TO HIS TRUCK WITH THE DULL BIT TO SHARPEN IT AND HE TRIPPED. HE FELL ADN THE DRILL BIT FELL ON HIS HAND.

HE WAS WORKING ON THE CAT 990 BUCKET LOADER. HE WAS HAMMERING THE BUCKET WITH HIS RIGHT HAND ANDA PIECE OF METAL BROKE OFF THE TOOTH AND HIT HIMIN THE UPPER LEFT ARM. EMPLOYEE WAS WEARING A WELDING SAFETY GARMENT.

HE SLIPPED ON THE STEPS AT THE TRAILER AT PRIMARY #1. HE HAD MUD ON HIS SHOES AND IT WAS RAINING. HE REPORTED THE INCIDENT AND TOLD THE SUPERVISOR HE MAY HAVE PULLED SOMETHING BUT REFUSED MEDI CAL TREATMENT. ON 5-24-00, HE SAID HE WAS STILL SORE AND HE WAS IMMEDIATELY TAKEN TO THE DOCTOR.

HE WAS REMOVING THE LOWER COVER ON THE HITACHI 1800 AND TWISTED HIS THUMB. HE INDICATED HE DID NOT NEED MEDICAL ATTENTION. ON 3/2/00 HIS THUMB WAS NOT BETTER AND HE WAS IMMEDIATELY TAKEN FOR M EDICAL TREATMENT.

ON 1-5-00 EE WAS IN THE CONTROL ROOM OF PRIMARY #2. WHEN HE OPENED THE DOOR DEBRIS PIECE OF WOOD FLEW IN HIS EYE DUE TO WIND CONDITIONS. HE TREID TO TAKE CARE OF IT HIMSELF BUT IT WAS BOTHERIN G HIM MORE SO HE REPORTED THE PROBLEM ON 1-7-00 HE WAS IMMEDICATELY TAKEN TO DR. FOR MEDICAL TREATMENT.

HE WAS CLIMBING ON THE LEFT REAR SECTION OF THE GROVE CRANE AND THE EXHAUST PIPE BROKE OFF CAUSING HIM TO FALL APPROX. 6' TO THE CONCRETE FLOOR IN THE SHOP.

EMPLOYEE WAS REHANGING UNDER THE SCREEN DECK AT PRIMARY #1. HE WAS MAKING A NEW HOLE AND THE KNIFE SLIPPED AND CUT HIS SECOND FINGER OF HIS LEFTHAND. FIRST AID WAS ADMINISTERED BUT STITCHES WERE REQUIRED.

EMPLOYEE WAS CLEANING AND REBUILDING CRUSHER #5 AT SECONDARY #1. WHILE GRINDING THE DUST AND BACKING POWDER, SOME FLEW IN HIS EYE. HE WAS PRESCRIPTION GLASSES BUT THE WIND WAS BAD. THEY TRIED TO FLUSH HIS EYE WITH EYE WASH BEFORE TAKING HIMTO THE NORTHEAST EYE INSTITUTE.

WHILE CUTTING IRON WITH A TORCH, A PIECE OF HOT IRON WENT DOWN HIS GLOVE. HE CLEANED AND PUT BURN CREAM ON HIS HAND. ON MONDAY, HE REPORTED HIS HAND WAS NOT GETTING BETTER, SO HE WAS TAKEN TO NORTHEAST OCCUPATIONAL MEDICINE FOR TREATMENT.

WHILE RUNNING SECONDARY #3 EE BUMPED AN ELECTRIC PLUG OUT OF THE SOCKET. EE TRIED TO GRAB IT AND PLUG IT BACK IN CAUSING A SHORT. THE SHORT SINGED THE HAIR ON HIS ARM AND A SMALL CUT ON HIS THUMB. FIRST AID WAS ADMINISTERED AND HE DECLINED TREATMENT AT NORTHEAST OCCUPATIONAL MEDICAL CENTER. AT ABOUT 11:00 AM HE REQUESTED TO SEE HIS DOCTOR BECAUSE HE WAS SHAKEN-UP.

HE LIFTED AN ELECTRIC MOTOR TO PUT IT IN THE CAT BACKHOE BY HIMSELF. HE WAS TOLD NOT TO LIFT IT HIMSELF.

HE WAS LUBRICATING THE 990 LOADER AND HIS FINGERS GOT COLD. HE MENTIONED TO EE HIS HANDS WERE COLD AND EE WANTED TO TAKE HIM TO THE MEDICAL CENTER BUT HE REFUSED. ON APRIL, HE TOLD EE HIS FI NGER WAS NUMB, WHITE, AND GIVING HIM TROUBLE. HE WAS TAKEN TO NORTHEAST MEDICAL CENTER, WHO DIRECTED HIM TO DR.

HE WAS HELPING THE MECHANIC FIX A FLAT TIRE. HE WAS HOLDING THE TIRE WHILE THE MECHANIC WAS USING THE BAR. THE BAR SLIPPED, CAME B ACK AND HIT HIM.

EE WAS OPERATING THE FRONT END LOADER AND BEGAN HAVING DISCOMFORT IN HIS BACK. HE CONTINUED WORKING. ON 11-4 HE REPORTED THE RPOBLEM TO OEPRATIONS MANAGER. ON 11-5 HE SAID HE WAS STILL HAVING LOWER BACK PAIN AND SLIGHT PAIN IN HIS LEGS AND WAS TAKEN TO DR. TO BE CHECKED.

ON 11/16/98 APPROX 9AM EE REPORTED TO QUARRY MASTER HE HURT HIS BACK 3 WEEKS AGO. HE SAID WHILE DRIVING TRUCK WHEN THE LOADER DUMPED ROCK INTO THE BODY OF HIS TRUCK IT JARRED HIS BACK. EE DID NOT REPORT IT IMMEDIATELY BECAUSE HE THOUGHT IT WOULD BE OK. UPON TALKING TO EE HE SAID THAT IT REALLY DID NOT HURT UNTIL THE MORNING OF 11/16/98

DIRT & DUST WAS BLOWING IN THE CAB OF HIS TRUCK THROUGHOUT THE DAY & HE CONT RUBBING THE EYE CAUSING IT TO BECOME IRRITATED.

HE WAS CLEANING THE CONVEYOR AND HE BUMPED HIS KNEE. HE DID NOT REPORT THE ACCIDENT UNTIL THE FLOLLOWING MORNING APPROXIMATELY 11:00 AM.
