Contractor was installing liner on the blade of dozer when a bolt broke that was supporting the metal plate. The plate dropped striking the contractor on the left foot. Result was a bruising and a fracture of minor bone in the left foot.

THE INJURY OCCURRED WHILE PRISING W/A SLATE BAR TO ALIGN AN ENGINE PART. THE BAR SLIPPED FROM ITS POSITION TO EE S HAND HIT EDGE OF BUMPER. INJURY CONSISTED OF 8 STITCHES. TO THE RIGHT THUMB A REA. 1 DAY LOST TIME INJURY.
AN EMPLOYEE ARRIVED ONT HE WORK SITE AT APPROX 5AM AND DISCOVERED THE DK40 HIGHWALL DRILL IN FLAMES. THE FLAMES WERE EXTINQUISED IN LESS THAN 30 MIN WITH DDRY CHEMICAL FIRE EXTINQUISHES.
