EE was changing a screen panel on the scalping screen, when EE pulled the panel out, EE heard a pop. EE was taken to after-hours where EE was then referred to the hospital with a torn left bicep and dislocated left shoulder which was later verified via MRI.

Worker slipped on wet step while descending step from loader, hit battery box with chest, and fractured three ribs. Worker returned to work the next day.

Worker is a retired, part-time crane operator occasionally called in to assist with crane work. Worker left crane to assist in belt replacement on short conveyor, climbed onto I-beam near conveyor, slipped and fell to ground. Worker landed on back. Injured back (compression fracture and general soreness).

Worker picked a propane tank out of back of pick-up truck to thaw frozen work area. Slipped and fell walking to work area. Initial diagnosis was contusions to ribs. Subsequent follow-up doctor visit 2/21 for check-up revealed fracture to rib.
