EE WAS OPERATING A PAYLOADER IN GRAVEL PIT. HE WAS DIGGING, PAYLOADER WAS BOUNCING, EE RECEIVED A PINCHED NERVE IN HIS LOWER BACK.

THREE EMPLOYEES STANDING ON THE FEEDER OF CRUSHER WERE REMOVING WORN JAW PLATE. THEY WERE USING A PAYLOADER AND A CHAIN FOR LIFTING THE JAW PLATE OUT OF CRUSHER TO SET ON WOODEN BLOCKS. JAW PL ATE SLIPPED INJURING FOOT. RESULT WAS A MULTIPLE FRACTURE ON THE RIGHT FOOT.
