Brooks Run South Mining, LLC · Struck against stationary object

After completing the cut in #4 heading near spad #3839 the employee was in a squat position. Upon standing and taking two steps, stepped onto the miner cable and waterline and twisted his right foot. Employee is unable to return to his regular duties.

Employee tossed a bundle of roof bolt glue sticks to a coworker on the other side of the machine when he felt a sharp pain in his back.

Employee was trimming a belt splice with a utility knife. The knife slipped and cut the top of his left hand.

Roof Fall was discovered during routine exam in the South East # 1 Sub Mains, in the number 2 entry near spad number 2513. The nearest working face is 3500 feet away from Fall area. Size of fall is 20 foot in length, 20 foot wide, and the height is 4 feet. All entrances to the fall area have been supported with additional supports, and access is blocked off.

Roof Fall was discovered around 7:30 am on morning of 07/31/2014. Size of the fall estimated at 20' X 20' X 4'. No one was injured and ventilation was not obstructed. All areas near the Fall have been cribbed off, and dangered off so no one can go in the effective area. The fall occured at Spad # 2485.

Roof Fall was discovered out by the working face of number one entry approximately 550' to 600' from the section The area was bolted according to roof control plan with four foot resin bolts. The area is in return air way doesn't block ventilation or prevent travel of the air course. All sides of the intersection have been cribbed off to prevent travel into the area.

Employee was removing drill steel from the drill pot when he felt a pain in his left shoulder.

The employee was starting to drill the outside bolt hole on his third row of bolts, as the steel started to rotate a piece of rock 30 1/2" X 14" X 2" fell from the top and struck his right foot resulting in a fracture. The employee was wearing all proper and necessary PPE.
