Miner has had an audiometric standard threshold shift in R-ear, pending retest, which results won't be available within the 10-day reporting criteria. Miner stated hobbies as music and guns, and worked with hearing protection directly before the initial test was performed.

The primary jaw had bridged over causing rocks to overflow the jaw. The injured miner was cleaning rocks off the deck and misjudged the distance between the rock in his hand and another rock. The result was a broken left index finger.

EE was inside the cone crusher inspecting the crusher for a problem and opened the hatch door to let some fresh air into the inspection area, after several minutes he forgot the hatch was open and leaned back and fell through the door and fell approximately 10 feet.

Employee fabricated a lifting device out of steel to lift a sheave for a v-belt drive assembly, upon lifting the sheave one of his weld broke and the sheave dropped a couple feet hitting the ground and rolling over his right foot, thus he incurred a bruised right foot needing a few days to recover.
