Replacing hoist jack on 480 Michigan loader. EE positioned himself on platform on front differencial housing w/pry bar to align eye on jack to be pinned. Bar slipped out of eye, causing EE to lose balance and fall from platform to ground, approx 5' landing on his left foot.

Laborer was loading his truck. He was climbing down the ladder from the bin he was loading from and hit his elbow on the cage around the ladder. The employee did not report the incident or see a doctor until 12/19/05. The office was not aware that this injury occurred at a mine until 1/6/05.

EMPLOYEE STEPPED FROM THE FUEL TANK OF THE PUMPERS TRUCK TO THE FUEL PLATFORM A DISTANCE OFF 3'4". HE SLIPPED AS HE MADE THE STEP FALLING A DISTANCE OF 6'2" FALLING BETWEEN THE PUMPERS TRUCK A DN THE FUEL STAND. HE STRUCK HIS LEFT ELBOW AGAIST THE STAND AS HE FELL. THIS CAUSED A FRACTURE TO HIS LEFT ELBOW.

EMPLOYEE THINKS CABLE CARRIER MIGHT HAVE QUIT RUNNING WHICH CAUSED LOSS OF POWER CAUSING CABLE CARRIER TO RUN OFF ROAD. EMPLOYEE TOOK OFF HIS SEATBELT TO JUMP OUT BUT DECIDED AGAINST JUMPING. HE WAS THROWN AROUND THE CAB CAUSING A BROKEN L1 AND L2 VERTEBRAE.

IN PIT AT THE FUEL TRUCK WITH D-9L DOZER CO. #03-8308. WAS GOING FROM DOZER TO FUEL TRUCK PLATFORM AND HIS FEET SLIPPED ON ICE, CAUSING HIM TO FALL TO GROUND LEVEL. WEATHER CONDITIONS: WAS FRE EZING RAIN AND ICE BUILT UP THROUGH OUT AREA DURING THE ENTIRE DAY.

HE GRABBED TH E EDGE OF THE CRAWLER PADS, WHICH CLAMPED DOWN ON HIS SMALL FINGER OF THE RT. HAND BETWEEN 2ND AND 3RD JOINT.

HE EXITED THE OPERATORS CAB ON L EFT SIDE AND SEEING NO EXIT, HE JUMPED FROM LEVEL OF CAB TO GROUND LEVEL WHICH IS 10'2". HE LANDED ON BOTH FEET AND THEN BACKWARDS ON HIS BACK. FIRE LASTED 20 MIN.

INJURED WAS HITTING PIN WITH SLEDGE HAMEMR TO DRIVE PIN INTO PAD, AND A PIECE OF METAL BROKE OFF TOP OF PIN, AND STRUCK INJURED IN UPPER INSIDE OF RIGH TLEG 6 INCHES ABOVE KNEE.
