EMPLOYEE WAS OPERATING THE BUCKET HOIST WHEN THE GLOVE ON HIS LEFT HAND GOT ENTANGLED IN THE PULLEY, RESULTING IN A LACERATION WHICH REQUIRED SUTURES AND A FRACTURE TO HIS LITTLE FINGERTIP.

EE WAS ASSISTING ANOTHER MAINTENANCE EE WHO WAS TRIMMING STEEL WITH A TORCH. THERE WAS A POP IN THE TORCH TIP CAUSING THE OTHER EE TO SUDDENLY MOVE IN REACTION. THE TORCH FLAME CAME CLOSE ENOU GHT TO EE'S GLOVE TO IGNITE THE CLOTH BACK OF THE GLOVE. HE TAKEN TOT HE CLINIC AND DIAGNOSED WITH 2ND DEGREE BURNS TO THE BACK OF HIS RIGHT HAND.

A TARPING STATION WITH FALL PROTE CTION IS PROVIDED AT THIS OPERATION. TRUCK DRIVERS ARE INSTRUCUTED TO USE THIS STATION FOR TARPING. WE REQUEST THIS INCIDENT NOT BE CHARGED TO THIS OPERATION

EE WHILE CHECKING HAMMER SCREEN, TWISTED HIS LEFT KNEE. DID NOT SEEK MEDICAL ATTENTION. ON 11/07 EE WAS DESCENDING BUCKET TRUCK LADDER. WHEN HIS FOOT CAME INTO CONTACT WITH THE GROUND HIS LEFT KNEE GAVE OUT ON HIM, AGGREVATING HIS 10/10 INJURING. ON 11/10 HE SOUGHT MEDICAL TREATMENT.
