While changing small motor, the motor smashed the employees finger.

On an icy day the loader operator started his loader and climbed down. The operator slipped and fell on ice. The manager helped him up the operator was okay. In August the employee reported pain associated with the fall, after being diagnosed with a degenerative issue claiming the fall aggravated his condition. September 15, 2011 employee had surgery to repair his condition.

EE WAS WALKING BETWEEN THE SAND PUMP AND SAND SCREW AND TRIPPED ON AN ELECTRICAL CONDUIT, WHEN THIS HAPPENED THE EE EXTENDED HIS LEFT ARM AND LANDED ON THE GROUND. HAD A SMALL FRACTURE TO LEFT ELBOW.

EE WAS TRAMMING THE R-50 HAUL TRUCK DOWN THE S. RAMP LOADED WHEN HIS BRAKES FAILED. HE DROVE TO THE NEXT BENCH THEN INTO THE BERM. THE TRUCK HITIT AT A PERPENDICULAR ANGLE CAUSING THE TRUCK TO JUMP THE BERM & STRIKE ANOTHER WHICH STOPPED THE TRUCK. THE OPERATOR WAS WEARING HIS SEAT BELT THUS KEEPING HIM IN THE TRUCK. X-RAYS SHOWED TWO FRACTURED RIBS. HE WAS RELEASED FOR LIGHT DUTY.

THE BAR SLIPPED CAUSING HIS LEFT PINKY FINGER TO BE SMASHED BETWEEN THE BAR AND A STEEL STRUCTURE. CO-WORKER WAS THE FIRST TO LOOK AT THE FINGER. HE TOOK HIM TO THE FIRST AID CABINET AND CLEANED IT UP AND PUT AN ICE PACK ON IT. HE THEN BROUGHT HIM TO ME IN THE SE
