An unintentional roof fall was found by mine examiner in the intake just outby spad 679.This fall was immediately called into MSHA because primary escape way blocked. No ventilation blocked. No injuries. Approximate dimensions 12x12x6.

An unintentional roof fall was found by mine examiner in the intersection of Spad 1399 in #7 Entry of Old 4 Right Mains. This fall was immediately called into MSHA because travelway blocked. No ventilation blocked. No injuries. Approximate dimensions 18x18x8. Mine in Non-Producing Status since 8/13/2013.

EE was hanging CM cable and sharp pain felt in right bicep.Scans revealed tear in tendon/muscle in right bicep.EE is going to physical therapy and surgery may be required.This report is filed late because individual whom normally filed 7000-1 reports was recently terminated and the person filing this report discovered on 7/15/2013 that it had not been reported.

Employee was bending roof bolts, he felt pain in his right shoulder. He was seen at the ER X-rays were taken and read Negative.

Employee was lifting a pull chain onto top step of a loader, he said he felt something pull in his right shoulder. He was taken to the ER for E-Rays.

Employee was loading a scoop ram jack onto a maintrip,he thought he pulled something in his hip. He was transported by ambulance to the ER,he was told he had a strain or tear in his back.

Employee was walking to the section ,he tripped over a belt head guard that was down in the mud. He was seen at med express where x-rays were taken and read negative.

Employee was hanging a brattice curtain behind the power center, he was standing on a crib block the crib block moved causing him to fall against the power center hitting his side and ribs. He was seen at the ER and xrays was taken and he was told he had a fractured rib.

Employee was unloading trash from a flat car and tripped on tongue of flat car. He was seen at the ER for a contusion to the leg.

Employee had came outside to fill out fireboss books, there was snow and ice on the ground, he was walking and slipped and fell. He was seen at summersville ER,where a x-ray was taken ,he was told that there was a tear in his Achilles tendon and there was a bone chipped in his left ankle.

Employee was shoveling at the No.2 belt head when he felt something pull in his right side. He did go to the ER and received a x-ray that was negative on 11-11-2012.

Employee had his hand laying on the tray of bolt machine watching the other operator set his bolt, when a rock fell hitting his left hand. He was taken to the ER and received an x-ray that was negative. He received four sutures in top of his left hand.

Employee was operating a shuttle car, he hit a hole in the roadway bounced him into canopy hitting his head. He received an x-ray at ER that was negative.

Employee was operating shuttle car, he hit a hole in the roadway, hitting his head on the shuttle car canopy.

Employee was lifting a belt guard into place. He turned to start a bolt in the guard twisting his knee.

Employee was riding in the maintenance man trip, There was some low roof, As they were moving he hit his head on a roof bolt.

Employee was using come a longs to unload shuttle car tire off mantrip. He was pulling on the come a longs when they got wedged, he was standing in mud. When he pulled, the come a longs broke loose causing him to twist his knee.

Employee was roof bolting, was bending roof bolt he felt something pull in right shoulder and his hand went numb. Was seen at Medical Center and received e-ray exam of shoulder.

Employee was shoveling belt line, shovel got caught between bottom roller and gob. The belt pulled the shovel inward causing his finger to hit the belt roller bracket. He went to ER and received Five sutures in left index finger.

Employee was roof bolting, a rock fell from the top between roof bolts and the rib tearing down line curtain. Employee was moving the rock off the curtain when he said that he felt something pull in his lower back.

Employee was roof bolting,The roof was high had to use alot of steel to reach top,he went to take bolt wrench down ,it fell hitting and fractured his ring finger.

Employee was lifting miner cable and water line to hang them from roof bolt,he felt something pull and pain in his back.He went to ER on 6-18-2012 had x-ray,he is scheduled for a MRI.

Employee was cleaning belt drive and take-up, he was shoveling gob into a wheel barrel. He was pushing the wheel barrel to dump it when he strained his left elbow. Employee was seen at the Er.

Employee was spot bolting,he bent down putting his knee on the mine floor,hitting his knee on a rock.Without knowing he stated his knee pad had slid down off of his knee.He went to the ER and it was treated.

Employee was roof bolting, he was pushing the bolt up with the head, the bolt bent causing the plate on the bolt to strike his left hand. He went to the ER and received five stitches in the palm of left hand.

A Unplanned Roof Fall Was Discovered On A Weekly Exam By Fireboss. The Location Of The Fall Was In The Left Return No.1 Entry Of The East Mains. Time Of Fall Is Unknown. This Was Reported On 2-28-2012 At 10:45 Am

Employee Was Getting Supplies Off Back Of Roof Bolter, he Had His Foot Under The Bolter When The Operator Lowered The ATRS Causing The Bolter To Come Down On Employee's Foot.Two Toes Were Fractured On The Left Foot.

A roof fall was discovered on a weekly exam in a abandoned area. No ventilation or travelways was affected time of fall is unknown, this was reported by fireboss at 10:20am on 01-19-2012. Location is one break inby spad 1167 in the north mains.

Employee had drilled a hole and was putting a wrench in the head of bolter when the steel fell out striking his little finger, there were three fractures to his right little finger

Using a hammer and punch to knock pin out of boom of miner, a piece of metal either came off of hammer or punch, becoming lodged in the right side of neck. Employee was seen at ER, then adivsed to have it surgically removed. Employee returned to work the next shift, metal was removed on 1-9-12, returned to work on 1-12-12.

Employee was unselective carrying metal roof straps (roof-bolting supplies) haphazardly in the supply area outby working section when he tripped over metal roof straps that were lying on mine floor. Employee fell down and injured his lower back as a result of tripping and falling.

Employee attempted to lift a bulky portable rock duster into scoop bucket by himself. The result was a Hernia in abdomen/stomach area. Employee had surgery to repair the hernia.

Employee and co-worker were hanging miner cable. As employee lifted the cable he stepped backwards and mistakenly stepped on co-workers foot causing the employees right knee to bend awkwardly and caused him to fall down on mine floor. Employee had an MRI on right knee and is waiting on results.

EE filed claim 6/27/11, no percentage has been granted. Person filing this report was unaware this needed to be reported without compensation being awarded. EE off full duty due to bypass heart surgery, not Occupational Pneumoconiosis related.

EE was operating #3 outby scoop traveling inby first set of equipment doors at mine portal when a piece of rock struck EE on top of his hard-hat and shoulder from mine roof/rib.

2 maint. emp. traveling on a 2-man trip on the surface area of the UG mine & the brakes failed. The trip rolled at the turn of the road. The passenger suffered fractures and is home recovering and anticipates returning to work asap. We anticipate the trip needs the following repaired; brake and susp. system, battery/electrical components.

EE did not report incident until 4/13/10. EE was installing 8 inch torque tension bolts and when he tried to straighten bolt he slipped and re-injured left elbow. EE sustained previous injury to left elbow at previous employer.

EE was supplying roof bolter on the 002-0 working section. EE reported he strained his back. EE went to doctor on 3/30/10 and RTW on RD on 3/31/2010.

EE was performing roof bolting operations and a small piece of rock fell and struck middle finger on right hand. EE received three sutures and RTW full duty on 3/30/10.

EE was walking in parking lot,tripped on gravel falling on left knee.EE reported falling awkward and twisted knee.EE worked until 4am at which time pain and swelling hindered walking.EE refused ambulance transportation.EE RTW on RD on 3/12/10. On 3/14/10 mine mgt. decided EE was not to RTW until released to full duty. EE quit on own terms on 3/21/2010.

EE was passenger on golf cart and not paying attention as golf cart was in motion, EE struck head on overcast located across track entry. Result was a laceration to forehead. EE was transported to emergency room and received six sutures.

An unintentional roof fall was found on mmu 002 working section in the intersection of number four entry one crosscut inby section loading point. The fall measured approximately 8-10' high, 20' length, 20' width. All approaches are cribbed and dangered off. The fall will not be cleaned up.

An unintentional roof fall above anchorage occurred in the 1st left panel of old mains. This area was mined by an operator/contractor prior to wvmp inc. The intersection @ spad 1880 fell and covered fresh water pump. At this time the fall will not be cleaned up.

An unintentional roof fall above anchorage was found in the old mains #5 entry. This area was mined by an operator/contractor prior to wvmp inc. At this time, the fall will not be cleaned up. Approaches have been timbered and dangered off.

EE was installing roof bolts in the #2 left crosscut on the 002-0 working section. A piece of rock fell from mine roof and struck EE on lower left forearm. EE sustained laceration and 8 sutures.

EE was changing hydraulic hose on #2 roof bolter when drill boom lowered compressing right arm between drill boom and frame. EE suffered a contusion to right arm below and above elbow area. X-rays were negative.

EE was loading Idle Roller into scoop bucket. The roller shifted in the scoop bucket making it roll towards where EE was standing. The roller landed on top of the right foot of the EE.

Alleged incident occurred while EE was attempting to change a drill pot on roof bolting machine. EE lifted drill pot to knee level when he felt a pain in his lower back. EE did not report incident or miss work until 8/18/2008 and also missed work on 8/19 and 8/20. EE did RTW on 8/21/2008. The person filing this report was made aware of the incident on 8/28/2008.

EE was moving miner cable in #1 entry when he stepped on rock and twisted right knee. EE sustained a sprained right knee. Compliance with rules/regs not a factor. EE awareness was a factor in the injury.

EE had finished his shift and was in the parking lot cleaning off the windshield of his truck. He fell against his truck after slipping on a piece of ice. EE sustained strained groin muscle. Cause of this injury was awareness. Compliance with rules/regs not a factor. Miner proficiency a factor. Miner attitude could be a factor.

EE was assisting miners in making belt splice at 3-head.EE was pounding nails into splice w/ hammer,hammer struck his left thumb.EE sustained dislocated thumb and needed 2 sutures.EE was released to RTW on 1/16/08.EE worked part of his shift on 1/16/08.EE went back to doctor and is not scheduled to RTW until 1/22/08.Miner proficency and awareness were factors in incident.

EE was roof bolting in 6 right on the active mmu 001-0, a piece of rock fell striking the canopy then bounced off striking the EE in the head and right foot. EE suffered a laceration to the head resulting in 5 sutures. Cause of this injury was awareness.Compliance with rules/regs not a factor, mining equip/systems not a factor, miner proficiency could be a factor.

EE was roof bolting in 5 right on the active mmu 001-0, a piece of rock binder on top of the coal seam loosened up from rib and struck the EE on the back of both lower legs. EE suffered a compressed right calf muscle. Cause of this injury was awareness.Compliance with rules/regs not a factor, mining equip/systems not a factor, miner proficiency could be a factor.

Employee was operating roof bolting machine from operator's compartment when he backed into the coal rib and pinched the trailing cable between the coal rib and the bumper of the machine. The employee allegedly received an electrical shock. Miner proficiency was a factor in the accident, mining equipment not a factor.

Employee was walking through a run-through check curtain and lost his balance and fell into the rib head first. Miner Proficency could be a factor, miner attitiude a factor, compliance with rules and regs not a factor.

An unintentional roof fall occurred approximatley 40 feet inby survey spad 1209. The fall measured approximatley 20' wide x 35' length x 1' to 10' feet high. The fall area has been cleaned up and re-supported with 8' fully grouted tension bolts.

When EE finished hammering the spad up , a rock fell & hit EE on the bridge of the nose. EE stopped & checked the roof. Then he hung the plum bob. While he was shining his light on the line another rock fell down and hit EE on back of hard hat. He then ran 25' or so and looked back and checked the roof.

Employee was loading roof straps on battery scoop car when hand slipped casuing a laceration on middle and ring finger on right hand. Employee claims he had leather gloves on. There were no witnesses to accident. Employee Proficency and Attitude factors in the accident.

Employee was holding straight edge while another employee was cutting belt with utility knife. The knife slipped and cut left pointer finger causing a laceration. Employee needed two stitches. Employee Experience a factor, Employee Proficinency a factor, Compliance with Rules and Regs not a factor.

Employee was installing crib blocks in the belt entry. A peice of rock measuring 12" inches x 6" inches x 4" thick fell from mine roof.He suffered a laceration on his forearm. Cause of injury was awareness. Mining equip/systems not a factor. Compliance with rules/regs not a factor. PPE not a factor. Miner attitude could be a factor.

A roof fall occured in the active working section, mmu number 001-0. The fall occured one break inby survey spad 6+96 in the intersection of number 6 entry and into the crosscut from 6 entry to 5 entry.

The accident was a roof fall in the #3 Entry of the active working section MMU-001. The 12-12 Remote Continious Miner was being operated at the time of the roof fall. The miner was taking his second cut out of X-cut 3 to 4.

Injured, a miner operator, was pulling miner cable when he pulled muscle in back.

While installing a roof bolt (spot bolt), a piece of rock (12" x 12" x 1 1/2") fell, striking injured on head, cutting forehead.

Injured was helping on miner, when a piece of rock was throws out of the boom, striking him in the hose.

INJURED WAS UNLOADING TIMBERS. WHILE REACHING FOR A TIMBER, ANOTHER EMPLOYEE THREW A TIMBER STRIKING HIM ON THE RIGHT MIDDLE FINGER. INJURED WAS WEARING GLOVES.

UNLOADING 100 LB. TOOL BOX FROM MAC 8, USING TWISTING MOTION, WITH HELP OF ONE CO-WORKER.

INJURED WAS HAMMERING ON DRILL STEEL (DRILL STEEL BEING USED AS A PUNCH) WHEN PIECE OF SMALL METAL HIT HIM IN RIGHT EYE. WITNESS STATED INJURED WAS WEARING SAFETY GLASSES AT TIME OF ACCIDENT. THIS OCCURRED WHILE REPLACING BITS IN CONTINUOUS MINER MACHINE.

INJURED WAS DRILLING HOLE & A PIECE OF ROCK, 2'X 2'X 3" FELL STRIKING HIM IN THE LEFT KNEE. INJURED WAS MEDICATED, SUPPORT BRACE APPLIED & RESTRICTED DUTY REQUIRED. MANAGEMENT WAS NOT SURE THIS WAS REPORTABLE.

Injured and contractor were changing down a tank that was being picked up by contractor. One side of the tank was properly secured and the other was being done when the cheater pipe slipped, allowing the binder to hit injured in the face (near right eye).

EE was loading fly boards onto scoop, slipped and fell, hurting lower back and tail bone.

WHILE PLASTERING PERM. STOPPING A ROCK FELL 12"L X 18" W X 2" THICK STRIKING EE ON UPPER BACK AND NECK AREA. STILL OFF WORK AS OF DATE OF THIS REPORT.

DURING A SECTION POWER MOVE. INJURED APPARENTLYHAD HANDS IN ELECTRICAL POWER BOX ON SECTION WHEN POWER WAS RESTORED FROM SURFACE, REENERGIZING THE BOX WITH 7200 VOLTS, CAUSING ELECTRICAL BURN S.

ROCK FALL 6' - 8' HIGH, 19' WIDE, 20' LONG @ #1 ENTRY. RETURN WAS TRAVELED 10-21-03 (12:30 PM).

A PIECE OF SLATE FELL OUT STRIKING THE OPERATOR IN THE NECK & SHOULDER AREA. IN A SUPPORTED AREA.

INJURED STATED HE WENT TO STEP OUT OF TRAILER WHEN HE SLIPPED AND FELL STRIKING HIS RIBS.

INJURED STATED HE HIT A HOLE WITH THE MANTRIP, INJURED LOWER BACK.

INJURED STATED DRILL STEEL WAS HUNG IN THE HOLY WHEN HE REACHED FOR A PEACE OF DRILL DRILL OFF THEN TRAY THE STEEL FELL FROM THE HOLE STIKING HIM ON THE TOP OF THE LEFT HAND.
