While dumping loaded truck, part of the load came out of the back of the trailer, when bed jack reached 2.5 to 3 stage the trailer went over onto the passenger side pulling tractor over onto its side, driver stated they were fine until they unbuckled seatbelt and wasn't able to hold their weight and fell to passenger side of truck.

A platform collapsed and fell approx. 28 feet. An employee on the platform was injured in the fall.
