ROOF FALL 45' INBY SPAD #47 200' OUTBY FACE OF #5 HDG. BOLTS WERE ANCHORED IN WILD COAL SLAIN. THE TOP FELL OTU AT THE TOP OF THE ANCHOR POINT FALL WAS 5' H 17'W 35'L. THE FALL WAS POSTED A ND DANGERED OFF AT BOTH POINTS OF ENTRY.

A ROOF FALL OCCURED IN #2 HDG 100' INBY SPAD #42. THE MINE WS IDLE FROM 11-20-92 TO 11-24-92. ON OUR FIRST SHIFT BACK WE DISCOVERED A ROOF FALL IN OUR #2 WORKING FACE ON OUR PRESHIFT EXAM. T HE FALL WHEN FROM THE FACE OUTBY FOR APPROX 30' JUST ABOVE THE 5' BOLTS BEING USED.

IN MAKING FIRE BOSS RUN I FOUND A FALL AT INTERSECTION OF SPADE AND POINT 12. THWERE WAS A CLAY VEIN GOING ACROSS INTERSECITON. T HIS AREA WAS CROSS BAR ADN TIMBERED AS WELLA S ANY PLACE IN A NY MINE. EXTRA TIMBERING WAS DONE WITH CRIBS SET TOO.
