EE was helping the continuous miner operator move the miner from the #5 entry to the #6 entry the miner operator stopped the miner to hook up a slack rope the miner operator hooked the rope to the boom of the miner went to turn around and stumbled and somehow hit the boom swing lever and somehow EE's hand was where the boom swung into the rib smashing his left hand.

An untentional roof fall has occurred in the 3rd East Main no. 3 entry outby 90 ft inby survey spad no. 2431. The fall measured 180 ft long, 18 ft wide, and 5 ft thick. The fall was above the anchor of installed roof bolts. The fall does not impair air movement, or impede travel in the area.
