WAS HAULING COAL ON SHUTTLE CAR TO DUMP; BOUNCED AND HIT ELBOW ON SIDE OF SHUTTLE CAR. WAS NOT REPORTED UNTIL 9-27-88.
WAS RUNNING 215C SHUTTLE CAR TAKING COAL TO DUMP ON TAILPIECE AND CAUGHT HIS HELMET ON A ROOF BOLT.
AN UNINTENTIONAL ROOF FALL OCCURRED AT THE #5 ENTRY (NEUTRAL ENTRY) INTERSECTION AT SPAD #33 3 BREAKS OUTBY THE #1 TAILPIECE IT WAS APPROXIMATELY 65' LONG, 20' WIDE, AND 50" THICK. IT WAS AT LEAST 1500' FROM THE FACE.
WAS MOVING POWER UP, PULLED MINER CABLE AND S TRIANED MUSCLE IN BACK.

UNINTENTIONAL ROOF FALL APPROX. 600' FROM FACE, 200' LONG 20' WIDE, 7' PLUS THICK. 1 1/2 BRAKES FROM #2 BELT LOCATION SPAD #134 1 BRAKE RIGHT HANDED, 1 BRAKE INBY, 1 BRAKE 20' X LEFTHANDED O FF INBY BRAKE. #1 ENTRY 28X20' 1 BRAKE INBY SPAD #132.
