LEDGE FOREMAN AND HIS LEDGE CREW WERE REMOVING STONE & DEBRIS FROM A BENCH LEVEL AT THE QUARRY SITE. THEY HAD REMOVED THE RAILING AND APPROXIMATELY HALF THE BENCH LEVEL AT THE TIME HE FELL 28 ' TO THE FLOOR BELOW THEM. HE PASSED AWAY 2/1/02.

HE WAS GOING TO MOVE A LITTLE PIPE BESIDE THE RADIATOR WHILE THE MOTOR ON THE WELDER WAS RUNNING. THE FAN ON THE WELDER CAUGHT HIS HAND AND INJURED SOME FINGERS.

HE SAID THAT HE WAS LOADING A ROCK WHEN HE JUMPED OFF THE TRUCK AND JARRED HIS SHOULDER. THE OTHERS AT THE QUARRY DID NOT SEE IT HAPPEN. THE DR SAID TO STAY OUT OF WORK WED. JULY 24TH THRU FRI DAY JULY 26, HE RETURNED TO WORK AROUND 1000 AM MONDAY, JULY 29TH BUT THEY DID NOT NEED HIM SO HE LEFT.
HE WAS HELPING CLEAN UP AT A QUARRY WHEN HE PICKED UP A ROCK AND HURT HIS BACK.HE WAS ALREADY HAVING SOME PROBLEMS WITH HIS BACK.HE WENT TO THE DOCTOR 02-28-91 AND THE DOCTOR SAID HE HAD A BAD PULLED MUSCLES AND SENT HIM FOR THERAPY TREATMENTS AND SAID NOT TO LIFT ANYTHING TOO HEAVY.HE WENT BACK TO WORK 03-04-91.THE DOCTOR RELEASED HIM 03-11-91.
