THE EMPLOYEE WAS HELPING PUT A NEW BELT ON A CONVEYOR. FIVE WORKERS WERE PULLING THE BELT ON WITH A ROPE & PULLEY. THE PULLEY HOOK BROKE AND THE PULLEY HIT HIM AND BROKE HIS RIGHT ARM.
