ASSUMING DIRT SHUTE ON ROCK CRUSHER WAS EMPTY, EE REMOVED SHUTE DOOR & INADVERTANTLY BENT OVER INSIDE TO CHECK LOWER BOTTOM SHUTE STOPPAGE. DIRTOVERHEAD FELL PINNING HIM ON THE SHUTE DOOR. EE NEVER LOST CONSCIOUSNESS. TALKED WHILE BEING REMOVED. PARAMEDIC ARRIVED ON SCENE WITHIN 3 TO 5 MINUTES. AMBULANCE ARRIVED WITHIN TEN.

TRUCK WAS LOADED AND BRAKES DID NOT HOLD. DRIVER JUMPED OUT AND TRUCK CONTINUED TO ROLL DOWN APPROX. 150'. DRIVER DROVE HIMSELF TO HOSPITAL AS PRECAUTION.

BAD JUDGEMENT BY OPERATOR IN SHIFTING GEARS ON AN 8 DEGREE GRADE. DRIVER MISSED A GEAR AND THEN APPLIED BRAKES. BUT AIR PRESSURE WAS AT ONLY 20 LBS. HE ROLLED BACKWARD DOWN GRADE APPROX. 50 YA RDS COMING TO AN ABRUPT STOP IN A DITCH. DRIVER SUFFERED A SONCUSSION AND A GASH TO HIS HEAD REQUIRING 24 STITCHES A CHEST CONTUSION AND VARIOUS BACK INJURIES.
