Improper use of hand tool. While unplugging a DTH, EE used a 48" pipe wrench as a hammer. While striking the DTH, the crook EE's right hand was pinched between the hook and handle causing a puncture injury requiring 3 stitches.

THE EMPLOYEE WAS INSTALLING A LINER PLATE IN A DUMP HOPPER. THE PEICE NEEDED SOME MODIFICAATION BEFORE IT COULD BE INSTALLED AND WAS SET ASIDE. THE PEICE WAS HELD BY A CABLE ATTACHED TO A BA R THROUGH A BOLT HOLE. THE PLATE SHIFTED, FELL AGAINST THE EMPLOYEE BREAKING HIS RIGHT LEGH.
