Associate picked up a steel plate & as associate was stacking the plate pinched the tip of finger.
Employee was climbing down from a D8 bulldozer, was standing on the tracks when foot slipped in the mud and employee fell off the track. Employee's wrist was lacerated by rock on the ground.

Employee was removing cutting edge modules from a 988k loader bucket. While loosening the bolt on the module, the bolt started to spin. Employee reached under and pushed up on the bolt, then engaged the air gun on the nut. This caused the bolt to slip and start spinning again. The bolt head tore through employee's glove into the palm, cutting EE's right hand.
DRILL TECHNCIAN WAS DEMO'ING A NEW DRILL. WHILE ATTEMPTING TO ADD STEEL LENGTH TO THE DRILL, TECHNICIAN'S HAND BECAME ENTANGLED IN THE LIFTING SLING. AS THE STEEL PIPE WAS LIFTED THE SLING PINCHED THE SMALL FINGER OF THE RIGHT HAND. TECHNCICAN REQUIRED MEDICAL CARE BEYOND FIRST AID.

WHILE REMOVING THE CUTTING EDGES FROM THE BUCKET OF THE 990 LOADER, ONE BOLT DROPPED CAUSING SPINNING. AS EE REACHED UNDER THE BUCKET, PUSHED THE BOLT BACK INTO THE SLOT & PULLED THE TRIGGER ON THE 1 INCH AIR IMPACT WRENCH, WHICH RESULTED IN CUTTING THROUGH EE'S GLOVE & LACERATING EE'S LEFT INDEX FINGER.
Employee was changing cutting edges on 990 loader. While using 1" impact gun, left hand slipped off the handle & middle finger came in contact with the socket & nut causing employee to get cut. Employee was taken to the hospital where employee received 10 stitches. Employee was wearing gloves.
While holding a wrench on a bolt to be removed on front suspension cylinder of 769C water truck, another associate was running an impact wrench. The nut loosened, retaining ring moved, & the socket flew-striking employee in the bridge of the nose.

Lightning entered the main breaker through a pole & then the transformer, causing a fire. Due to the electrical storm, the power company could not shutdown the power & the fire department could not enter the building until the power was shutdown.

EE was standing on bed of welding truck w/ 3 screens in front of him, leaning on welder. He pushed 1st screen onto EE's forks & screen caught his glove, pulling him off truck. He then fell to the ground and during the fall, the sharp edges of screen cut his arm, chin & neck. The arm injury was severe, with his skin being peeled back exposing muscle, veins and possibly tendons.
Associate, replacing broken spring on FMC feeder in Tunnel A at 3M secondary, was using chain come-a-long. While pushing comealong, pulled muscle in back.

Associate was installing middle wedge on stationary liners at C145 jaw crusher. He slipped and accidently hit the release tab on the safety bar holding liner plate. Plate then tipped forward pinning associate's legs between the stationary liner and the swing jaw.
Assoc installing fan belts on pulley on equip with assistance from another mechanic with a bar, the bar slipped releasing the tensioner pulley, Assoc finger caught between pulley and fan belt.
Assoc. replacing bolds on hydraulic manifold located under lever that operates blades, hit elbow hard against bracket holding hydraulic hoses

Associate was cutting a plastic strap from a box of blasting caps. The knife had a new blade and cut thru the strap with more force than expected. Sliced thru associates pants into leg requiring 4 stitches.

A SECTION OF CATWALK FLOOR GRATING HAD NOT BEEN INSTALLED WHERE TWO SECTIONS OF CONVEYOR HAD JOINED TOGETHER. EE HAD FAILED TO PUT HIS SAFETYHARNESS ON AFTER HE TOOK A BREAK. HE WAS INSTALL ING HANDRAILS ON THE CATWALK IN THE AREA OF THE OPENING AND FELL THROUGH THE HOLE.
ASSOC FUELING 775D HAUL THERE WAS ANOTHER VEHICLE IN WAY. DIDN'T ASK TO MOVE STRETCHED FUELING HOSE TO UNIT. WHEN PUMP DISENGAGED HOSE PULLED OUT OF VEHICLE SPLASHING ON ASSOC & IN EYES.
ASSOCIATE CLIMBING OFF WATER TRUCK STEPPING DOWNFOOT CAUGHT IN SWINGING STEP. HE FELL, GRABBED STEP TO CATCH SELF. STEP SWUNG CRUSHING RIGH MIDDLE FINGER.
WHILE LOWERING DOLLIES ON FLATBED, TRAILER HANDLE KICKED BACK, STRIKING EE IN FOREHEAD.
